摘  要:目的评价七氟醚后处理(sevoflurane postconditioning,SPC)对大鼠心肌缺血再灌注损伤中线粒体释放活性氧簇(reactive oxygen species,ROS)的影响。方法雄性SD大鼠36只,体重250~300 g,采用随机数字表法分为3组:假手术组(Sham组)、缺血再灌注组(I/R组)和七氟醚后处理组(SPC组),每组12只。心肌缺血/再灌注(I/R)损伤模型建立应用Langendorff离体心脏灌流装置。Sham组持续灌注重碳酸盐缓冲液(Krebs-Henseleit,K-H)170 min;I/R组平衡20 min,后全心灌注4℃St.Thomas停跳液,32℃下缺血30 min,后用K-H溶液灌注120 min;SPC组平衡20 min,同I/R组条件下缺血30 min,后用1.0最低肺泡有效浓度(minimum alveolar concentration,MAC)的七氟醚饱和的K-H溶液全心灌注15 min,后连续灌注K-H溶液105 min。测定线粒体ROS产生率(RPR)和ATP含量。再灌注结束时,取5只大鼠心脏,测定心肌梗死面积。结果与Sham组比较,I/R组心肌梗死面积增加,ROS生成增多,ATP合成减少(P<0.05);与I/R组比较,SPC组心肌梗死面积减小,ROS生成减少,ATP合成增加(P<0.05);Sham组和SPC组上述指标差异无统计学意义(P>0.05)。结论大鼠心肌缺血再灌注时,七氟醚后处理抑制线粒体ROS产生,减少心肌梗死面积,并增加ATP合成。Objective To evaluate the effects of sevoflurane postconditioning(SPC)on mitochondrial release of reactive oxygen species(ROS)during myocardial ischemia-reperfusion injury in rats.Methods Thirty-six male SD rats weighing 250-300 g were divided into sham operation group(Sham group),ischemia-reperfusion group(I/R group)and sevoflurane post-treatment group(SPC group)with 12 for each group.Myocardial ischemia/reperfusion(I/R)injury model was established using Langendorff isolated heart perfusion device.The sham group was continuously perfused with Krebs-Henseleit solution(K-H)for 170 minutes;the I/R group was equilibrated for 20 minutes,and then the whole heart was perfused with 4℃St.Thomas cardioplegia,ischemia at 32℃for 30 minutes,and then perfused with K-H solution for 120 minutes;the SPC group was equilibrated for 20 minutes,and ischemic for 30 minutes under the same I/R group conditions,then the heart was saturated with 1.0 minimum alveolar concentration(MAC)sevoflurane-saturated K-H solution for 15 minutes,and then continuously perfused with K-H solution for 105 minutes.Mitochondrial ROS production rate(RPR)and ATP content were measured.At the end of reperfusion,5 rat hearts were taken and the myocardial infarct size was measured.Results Compared with the sham group,the myocardial infarct size increased,ROS production increased,and ATP synthesis decreased in the I/R group(P<0.05).Compared with the I/R group,the myocardial infarct size decreased,ROS production decreased,and ATP synthesis increased in the SPC group(P<0.05);There was no significant difference between the sham group and the SPC group(P>0.05).Conclusion After myocardial ischemia-reperfusion in rats,SPC inhibited mitochondrial ROS release,reduced myocardial infarct size,and increased ATP synthesis.
