摘 要:目的探讨表皮细胞生长因子(EGF)与肛瘘术后创面愈合速度的相关性以及重组人EGF的临床疗效。方法选取2018年9月至2019年9月嘉兴市第二医院收治的79例单纯性肛瘘术后创面难愈患者为研究对象,定期采集患者创面渗出液测定创面EGF水平,使用数码相机对患者创面进行拍照,经PHOTOSHOP CS软件处理后测定创面愈合速度,分析两者的相关性。肛瘘术后第4周患者创面仍未完全愈合28例,为慢愈合组;剩余51例为一般愈合组。对慢愈合组患者予以重组人EGF喷涂创面(睡前1次/d),观察治疗前后创面愈合速度变化。结果患者肛瘘术后第1~4周创面EGF水平与创面愈合速度均呈正相关(均P<0.05),慢愈合组创面EGF水平及愈合速度均低于一般愈合组,差异均有统计学意义(均P<0.05)。使用重组人EGF喷涂后,慢愈合组患者第5~6周创面愈合速度较前4周明显增加,差异有统计学意义(P<0.05)。结论肛瘘术后创面EGF水平与愈合速度呈正相关;涂抹重组人EGF能加快创面愈合,值得在临床推广应用。Objective To investigate the correlation between epidermal growth factor(EGF)and wound healing of anal fistula.Methods Postoperative tissue and exudatesampleswere collected from 79 patients with anal fistula undergoing surgical treatment.The EGF content was detected with ELISA,the anal fistula wound was photographed and the wound healing rate was calculateas wound area per week(cm^2/week)with PHOTOSHOP.The correlation between the wound healing rate and the EGF content in the wound was analyzed.According the wound healing 4 weeks after operation,28 cases were classified as slow healing group and 51 cases were classified as general healing group.In the slow-healing group,the wounds were sprayed with recombinant human EGF(rhEGF)qd before bed,and the healing speed of the wounds was observed before and after treatment.Results Postoperative healing rate was positively correlated with the EGF content in the wound surface from the first to fourth week after the operation(P<0.05).The EGF level and healing speed of wounds in the slow-healing group were lower than those in the general healing group(P<0.05).Treatment with rhEGF,the wound healing rate was significantly accelerated at week 5-6 compared with that in the first 4 weeks(P<0.05).Conclusion There is a positive correlation between the level of EGF and the rate of healing after anal fistula surgery.Application of recombinant human EGF can accelerate wound healing and is worthy of clinical application.
