白假丝酵母耐药蛋白-1表达上调与氟康唑耐药关系研究

摘  要:目的:探究白假丝酵母耐药蛋白-1 (CDR-1)蛋白表达上调与氟康唑耐药的关系。方法:采用常规真菌分离鉴定法,分离鉴定46株白假丝酵母;采用微量稀释法测定氟康唑对各株白假丝酵母的最低抑菌浓度(MIC),依据氟康唑MIC值将白假丝酵母分为3组(氟康唑敏感组、剂量依赖敏感组、耐药组);采用Western blot法检测各菌株CDR1蛋白表达水平,运用Person相关法分析白假丝酵母CDR-1蛋白表达水平与氟康唑MIC的相关性。结果:耐药组、剂量依赖敏感组、敏感组白假丝酵母的氟康唑MIC值中位数分别为128μg/mL、16μg/mL和1.5μg/mL;耐药组白假丝酵母的氟康唑MIC值高于剂量依赖敏感组和敏感组(P<0.05);耐药组、剂量依赖敏感组、敏感组白假丝酵母CDR-1蛋白表达相对倍数分别为6.15±1.97、1.69±0.27和1.12±0.09,耐药组白假丝酵母CDR1蛋白表达水平相对倍数高于剂量依赖敏感组和敏感组(P<0.05),剂量依赖敏感组白假丝酵母CDR-1蛋白表达水平相对倍数高于敏感组白假丝酵母(P<0.05);耐药组白假丝酵母的CDR-1蛋白表达水平与对数转换后的菌株氟康唑MIC值呈正相关(r=0.773,P<0.05)。结论:白假丝酵母CDR1蛋白表达上调是白假丝酵母对氟康唑耐药的重要因素,CDR-1是防止白假丝酵母对氟康唑耐药扩散的潜在靶点。Objective: To study the relationship between up-regulation expression of Candida albicans(C.albicans)drug-resistant protein-1(CDR-1) and C.albicans resistance to fluconazole(FLU). Methods: After 46 strains of C.albicans were isolated and identified from clinical samples by regular fungal isolation method, and the minimum inhibitory concentration(MIC) of C.albicans to FLU was determined by microdilution method. Based on the MIC level of C.albicans to FLU, 46 strains of C.albicans were divided into 3 groups, including FLU-resistant strain(R group),dose dependent FLU-sensitive strain(DS group), and FLU-sensitive strain(S group). The expression level of CDR-1 protein in each C.albicans strain was detected by Western blot, and the person correlation was used to analyze the relationship between the expression level of CDR-1 protein and MIC of C.albicans to FLU. Results: The median MIC values of C.albicans to FLU in R group, DS group and S group were 128 μg/mL,16 μg/mL and 1.5 μg/mL, respectively. The median MIC value of C.albicans to FLU in R group was higher than those in DS group and S group(all P<0.05). The relative level of CDR-1 protein expression of C.albicans in R group, DS group and S group were 6.15±1.97, 1.69±0.27 and 1.12±0.09, respectively. The relative level of CDR-1 protein expression of C.albicans in R group was higher than that of DS group and the S group(both P<0.05), and The relative level of CDR-1 protein expression of C.albicans in DS group was higher than that in S group(P<0.05). There was a positive relationship between the relative level of CDR-1 protein expression and the Logarithmic transformation of MIC value of C.albicans to FLU(r=0.773, P<0.05). Conclusion: The up-regulation expression of CDR-1 protein in C.albicans is an important factor for C.albicans resistance to fluconazole, and CDR-1 protein is a potential target to prevent the spread of C.albicans resistance to fluconazole.
