甲状腺全切除术在甲状腺良性疾病患者首次手术治疗中的应用效果

摘　　要：目的探究甲状腺全切除术在甲状腺良性疾病患者首次手术治疗中的临床效果。方法随机选取2017年4月至2019年4月于本院首次接受手术治疗的88例甲状腺良性疾病患者,并按照双盲法分为两组,对比组实施甲状腺次全切除术,试验组予以甲状腺全切除术治疗,对两组患者的手术时间、术中出血量、并发症发生率与复发率进行比较。结果试验组患者手术时间与术中出血量分别为(79.01±19.19)min、(57.96±20.05)mL,明显少于对比组的(96.71±22.89)min、(110.29±29.84)mL(t_1=3.9307,P_1=0.0002;t_2=9.6555,P_2=0.0000);试验组并发症发生率为4.54%,对比组为9.09%,两组比较,无统计学意义(χ^2=0.7154,P=0.3976);对比组患者随访12个月复发率20.45%,高于试验组的4.55%(χ^2=5.0909,P=0.0241)。结论甲状腺全切除术对甲状腺良性疾病患者给予首次手术治疗,可减少手术时间与术中出血量,降低患者的复发率。Objective To explore the clinical eff ect of total thyroidectomy in the fi rst operation of patients with benign thyroid diseases.Methods Randomly selected 88 patients with benign thyroid disease who received surgery for the fi rst time f rom April 2017 to April 2019,and divided them into two groups according to double-blind method.The comparison group underwent subtotal thyroidectomy,the experimental group was treated with total thyroidectomy.The operation time,intraoperative bleeding,complication rate and recurrence rate were compared between the two groups.Results The operation time and intraoperative blood loss of the experimental group were(79.01±19.19)min and(57.96±20.05)mL,respectively,which were significantly less than(96.71±22.89)min and(110.29±29.84)mL in the comparison group,(t1=3.9307,P1=0.0002;t2=9.6555,P2=0.0000);the complication rate in the experimental group was 4.54%,and the comparison group was 9.09%.There was no statistical signifi cance between the two groups(χ^2=0.7154,P=0.3976);patients in the comparison group the recurrence rate was 20.45%in the 12-month follow-up,which was higher than the 4.55%in the experimental group(χ^2=5.0909,P=0.0241).Conclusion Total thyroidectomy for patients with benign thyroid disease to give the fi rst surgical treatment,can reduce the operation time and intraoperative bleeding,reduce the recurrence rate.
