超声心动图对老年钙化性心脏瓣膜病的诊断价值

摘　　要：目的分析超声心动图对老年钙化性心脏瓣膜病的诊断价值。方法选择我院于2018年1月至2019年2月收治的510例老年钙化性心脏瓣膜病患者的超声心动图资料进行回顾性分析,观察并记录患者声像学资料的特点以及对该病的诊断价值。结果超声心动图声像学显示,351例(68.82%)单纯主动脉瓣受累,62例(12.16%)单纯二尖瓣受累,97例(17.84%)二者均受累;患者瓣膜本身、边缘存在回声增强的情况,严重者瓣膜基底部存在团状强回声情况,瓣膜的表面毛糙且活动受限,闭合不佳且合并反流;203例(39.80%)主动脉瓣反流及117例(22.94%)主动脉瓣狭窄、120例(23.53%)二尖瓣反流及70例(13.73%)二尖瓣狭窄,多为轻中度情况。95例(18.63%)左心室扩大、287例(56.27%)左心房扩大、90例(17.65%)左心室心肌肥厚及58例(11.37%)全心扩大;120例(23.53%)左心室收缩功能下降,402例(78.82%)左心室舒张功能下降,LAD平均值为(48.79±4.23)mm、LVD平均值为(52.32±3.57)mm、LVEF平均值为(57.25±10.78)%。结论针对老年钙化性心脏瓣膜病的诊断,应用超声心动图效果尤为显著,能够明确观察到患者的心脏功能、形态及钙化情况,在临床上值得应用推广。Objective To analyze the diagnostic value of echocardiography in elderly patients with calcifi ed valvular heart disease.Methods from January 2018 to February 2019,510 cases of elderly patients with calcifi ed valvular heart disease were selected for retrospective analysis.The characteristics of ultrasonic data and the diagnostic value of the patients were observed and recorded in detail.Result Echocardiography showed that 351 cases(68.82%)had aortic valve involvement,62 cases(12.16%)had mitral valve involvement,and 97 cases(17.84%)had both;in severe cases,echo enhancement was found in the valve itself and the edge;in severe cases,there was mass strong echo in the valve base;the surface of the valve was rough and limited,poor closure and refl ux;203 cases(39.80%)were involved aortic regurgitation and aortic stenosis were found in 117 cases(22.94%),mitral regurgitation in 120 cases(23.53%)and mitral stenosis in 70 cases(13.73%).There were 95 cases(18.63%)with left ventricular enlargement,287 cases(56.27%)with left atrial enlargement,90 cases(17.65%)with left ventricular hypertrophy and 58 cases(11.37%)with whole heart enlargement;120 cases(23.53%)with decreased left ventricular systolic function,402 cases(78.82%)with decreased left ventricular diastolic function,mean lad was(48.79±4.23)mm,LVD was(52.32±3.57)mm,and LVEF was(57.25±10.78)%.Conclusion For the diagnosis of senile calcifi ed heart valve disease,the application of echocardiography is particularly signifi cant,which can clearly observe the heart function,shape and calcifi cation of patients,which is worthy of clinical application and promotion.
