早期显微手术夹闭瘤颈治疗脑动脉瘤破裂出血患者的疗效观察及对预后的影响

摘　　要：目的探讨早期显微手术夹闭瘤颈治疗脑动脉瘤破裂出血患者的疗效及对预后的影响。方法选取2017年2月至2019年2月本院脑外科收治的62例脑动脉瘤破裂出血患者,按照随机数字表法分为对照组和研究组,每组31例。两组患者在发病早期(出血72 h内)接受手术抢救治疗,对照组采用早期介入手术治疗,研究组患者进行早期显微手术夹闭瘤颈治疗,比较两组患者的预后良好率以及并发症发生率。结果研究组患者中预后良好26例,预后不良5例;对照组中预后良好11例,预后不良17例;研究组的预后良好率(83.87%)明显高于对照组(35.48%),差异有统计学意义(P<0.05)。研究组并发症发生率(3.22%)与对照组并发症发生率(9.67%)比较差异无统计学意义。结论早期显微手术夹闭瘤颈治疗脑动脉瘤破裂出血患者疗效显著,且不良反应发生率低,安全性较高,值得临床研究和推广。Objective To investigate the effect of early microsurgical clipping of the neck on the treatment of cerebral aneurysm rupture and its prognosis.Methods 62 patients with cerebral aneurysm rupture and hemorrhage admitted to our hospital were enrolled from February 2017 to February 2019.They were divided into control group and study group according to the random number table method,with 31 cases in each group.The patients in the two groups were treated with surgery in the early stage of the disease(within 72 hours after bleeding).The control group was treated with early interventional surgery.The patients in the study group underwent early microsurgical clipping of the neck.The good prognosis rate and the incidence of complications were compared between the two groups.Results In the study group,26 patients had a good prognosis and 5 patients had a poor prognosis.In the control group,11 patients had a good prognosis and 17 patients had a poor prognosis.The good prognosis rate(83.87%)of the study group was significantly higher than that of the control group(35.48%),and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the study group(3.22%)and the control group(9.67%).Conclusion Early microsurgical clipping of the neck is effective in the treatment of cerebral aneurysm rupture,and the incidence of adverse reactions is low.It has high safety and is worthy of in-depth research and promotion in clinical practice.
