产前超声筛查在中孕期胎儿复杂先天性心脏畸形中的诊断价值分析

摘　　要：目的:研究产前超声筛查在中孕期胎儿复杂先天性心脏畸形中的诊断价值。方法:2018年9月-2019年9月收治进行胎儿检查的产妇1200例,均进行产前超声诊断,查看胎儿超声心动图,了解胎儿是否出现心血管疾病,分析产前超声筛选的诊断价值。结果:所有产妇通过产前超声诊断发现心脏畸形胎儿45例,在产后检查及随访中发现心脏畸形胎儿48例,产前超声筛选准确率为93.75%,漏诊率为6.25%,所有心脏畸形胎儿分为完全性心内膜垫缺损25.00%、法洛四联症16.67%、右室双出口14.58%、静脉异位引流12.50%、大动脉转位10.42%、永存动脉干10.42%、主动脉弓离断6.25%、其他8.33%。所有心脏畸形胎儿主要分为完全性心内膜垫缺损、法洛四联症及右室双出口。结论:中孕期产妇进行产前心脏超声筛选能有效诊断胎儿是否存在心脏畸形,具有较高的诊断价值。Objective:To analyze the diagnostic value of prenatal ultrasonographic screening for complex congenital heart disease in the fetus during the second trimester.Methods:From September 2018 to September 2019,1200 women who underwent fetal examination were selected.All of them were diagnosed with prenatal ultrasound,and fetal echocardiography was performed to determine whether the fetus had cardiovascular disease.We analyzed the diagnostic value of prenatal ultrasound screening.Results:45 cases of fetal heart malformation were found in all the parturients through prenatal ultrasound diagnosis,48 cases of fetal heart malformation were found in the postpartum examination and follow-up,the accuracy rate of prenatal ultrasound screening was 93.75%,the missed rate was 6.25%,all the fetal heart malformations were divided into complete endocardial cushion defect rate of 25.00%,tetralogy of Fallot rate of 16.67%,right ventricular double outlet rate of 14.58%,the proportion of venous ectopic drainage was 12.50%,the proportion of transposition of great arteries was 10.42%,the proportion of permanent trunk was 10.42%,the proportion of aortic arch disconnection was 6.25%,and the other proportion was 8.33%.All fetal heart malformations were divided into complete endocardial cushion defect,tetralogy of Fallot and right ventricular double outlet.Conclusion:Prenatal cardiac ultrasound screening in mid-pregnant women can effectively diagnose the presence of cardiac malformations in the fetus,which has high diagnostic value.
