烧伤创疡再生医疗技术在自体动静脉内瘘感染中的应用效果分析

摘　　要：目的观察分析烧伤创疡再生医疗技术治疗维持性血液透析患者自体动静脉内瘘感染的临床疗效。方法选取2016年1月至2019年6月哈尔滨市第四医院收治的6例维持性血液透析过程中出现自体动静脉内瘘感染的患者作为研究对象,给予其抗感染、改善微循环、营养支持等全身综合治疗的同时,局部创面采用烧伤创疡再生医疗技术治疗,观察治疗效果。结果 4例穿刺部位非开放性感染患者中有3例患者经切开引流及湿润烧伤膏换药治疗后15 d内创面完全愈合,1例患者因血管扩张呈瘤样而转为外科手术治疗;2例瘘口开放性感染患者分别于湿润烧伤膏换药治疗27 d及42 d后创面愈合,动静脉内瘘功能恢复良好。结论烧伤创疡再生医疗技术可有效控制自体动静脉内瘘的局部感染,促进创面愈合,疗效显著,且操作简便,临床应用价值较高。Objective To observe and analyze the therapeutic effect of MEBT/MEBO in the treatment of autologous arteriovenous fistula infection in patients undergoing maintenance hemodialysis.Methods Six patients,admitted to Harbin Fourth Hospital from January 2016 to June 2019 and developed autologous arteriovenous fistula infection during the process of maintenance hemodialysis,were selected as research subjects.In addition to systemic comprehensive treatments,such as anti-infection,improvement of microcirculation and nutritional support,the patients were also given the local treatment with MEBT/MEBO.And the treatment effect was observed.Results Among the 4 patients with non-open wound infections at the puncture sites,3 patients were treated with incision and drainage,and the wounds were cured after the dressing change with MEBO for 15 days,and the blood vessels of the other one patient expanded like a tumor,and thus the patient underwent a surgical operation to remove it.The two patients with open fistula infection were treated with MEBO dressing change for 27 and 42 days respectively,and their wounds were healed and the functions of their arteriovenous fistulas recovered well.Conclusion MEBT/MEBO can effectively treat the local infection of autologous arteriovenous fistula and promote wound healing,presenting a high value of clinical application given its easy and simple use and good therapeutic effect.
