摘 要:目的:构建具有精细结构的真实膝关节三维模型,分析不同生理状态下人体电学信号的变化,为膝部积液检测系统设计提供理论指导。方法:基于人体下肢CT图像,构建具有膝关节真实结构的三维模型,并在其基础上构建膝关节积液生物阻抗检测模型,利用有限元仿真分析不同积液程度、不同积液位置时膝关节模型的电场分布,并设计琼脂实验对膝关节三维模型的有效性进行验证。结果:仿真结果表明,膝关节表面电学信号受积液变化影响,积液电导率由0.80 S/m增至1.60 S/m时,电势值减小约0.06 mV,阻抗值减少约4Ω;积液位置不同时,积液区域内的电势变化率最大约为0.02 mV。琼脂实验结果表明,积液电导率由0.80 S/m增至1.60 S/m时,阻抗值减少约40Ω。仿真结果和实验结果中阻抗的变化趋势一致,均随积液电导率增加而减小。结论:积液程度的加重会引发生物体电势值和阻抗值的减小,局部积液产生会导致该区域电势值小于其他区域。该研究为膝关节疾病的早期诊断和术后康复治疗提供了理论依据。Objective To construct a three-dimensional model of a real knee joint with a fine structure,analyze the changes of human body electrical signals under different physiological conditions,and provide theoretical guidance for the design of knee effusion detection system.Methods CT images of human lower limbs were used to construct a three-dimensional model of the real structure of the knee joint.A bioimpedance model was designed to simulate and analyze the electric field distribution of knee joint model with different effusion degrees and different effusion positions.An agar experiment was developed to compare the experimental results with the simulation results to study the bioimpedance effusion detection method of knee joint.Results The electrical signals on the surface of the knee joint were affected by changes in the effusion.When the conductivity of the effusion increased from 0.80 S/m to 1.60 S/m,the potential value decreased by about 0.06 mV,and the impedance was reduced by about 4Ω.When the fluid accumulation occurred at different positions,the maximum potential change rate in the fluid accumulation area was about 0.02 mV.The results of agar test showed that the impedance value was lowered by about 40Ω,and the simulation and experimental impedance values were reduced with the increase of conductivity.Conclusion The simulation model provides a theoretical basis for early diagnosis and postoperative rehabilitation treatment of knee joint diseases,and the research results lay a foundation for the design of knee joint bioelectrical impedance detection system.[Chinese Medical Equipment Journal,2020,41(10):7-12,23]
