高频超声检测腕管段永存正中动脉及其临床意义

摘　　要：目的探讨高频超声对腕管段永存正中动脉的检测价值及其在腕管综合征诊治中的意义。方法对300例健康志愿者的600只手腕部腕管结构及前臂进行高频超声检查。动态观察正中神经有无伴行血管,发现伴行血管时,利用彩色多普勒、频谱多普勒及探头加压等方法确认动、静脉,并记录腕管段血管直径、血管来源等情况。结果300例受检者600只手腕部扫查中,共发现永存正中动脉阳性者12侧,最小直径约0.05cm,最大直径约0.19cm,平均直径约0.12cm。永存正中静脉阳性者8侧,均伴随永存正中动脉出现。结论高频超声不但可以清晰显示腕管段永存正中动、静脉,而且能准确诊断血栓、血管瘤、管壁钙化等病理情况,为腕管综合征的诊断提供全面、直观的信息,有助于选择合理的外科手术入路,避免损伤血管及解压不完全等并发症。Objective To investigate the value of high resolution ultrasonography in demonstrating the anatomical variations of the median vessels in carpal tunnel and its clinical significance.Methods A total of 600 hands of 300 healthy volunteers were included in the study.The location and shape of the persistent median artery and persistent median vein were recorded.The diametes of the persistent median artery in carpal tunnel were measured.Results Among the 600 wrists examined,persistent median artery was observed in 12 wrists,the minimum diameter was about 0.05 cm,the maximum diameter was about 0.19 cm,the average diameter was about 0.12 cm.Persistent median vein was observed in 8 wrists,both appearing with persistent median artery.Conclusion The anatomical variations of the median vessels in carpal tunnel could be well depicted using high resolution ultrasonography.Identification of these variations may contribute to the full and accurate diagnosis of carpal tunnel syndrome and reasonable choice of surgical method and approach,avoiding the complications of nerve and vessels injury and incomplete decompression.
