血小板直方图异常时三种检测方法的比较研究

摘　　要：目的在血小板直方图或形态异常时比较三种血小板检测方法,选择最佳方式用于临床。方法分别采用电阻抗法、光学法、电阻抗-拟合曲线法对35例血小板直方图未报警标本、100例仪器提示“血小板异常直方图”标本进行检测并采用统计学分析及利用瑞姬染色法进行复检。结果100例提示“血小板异常直方图”标本中12%为血小板聚集造成、18%为小红细胞影响、其余70%为大血小板影响;实验组50例,14%为小红细胞影响、86%为大血小板影响。仪器无血小板报警时三种方法检测血小板结果差异无统计学意义(P>0.05);仪器提示“血小板异常直方图”时三种方法检测血小板结果差异有统计学意义(P<0.05)。结论血小板直方图异常时,需要复检,必要时需要与临床医师沟通;血小板直方图异常时,可参考光学法检测血小板。Objective To compare three different detection methods of platelet with abnormal histogram or morphology and identify the best method for the clinical diagnosis and treatment.Methods Electric resistance platelet count(PLT-I),optical platelet count(PLT-O)and electric resistance-fitting platelet count(PLT-B)were applied to detect 35 normal samples and 100 abnormal samples of platelet histogram.The results were analyzed and reviewed by statistical methods and W-J staining.Results Among 100 patient specimens with abnormal platelet histogram collected,12%were caused by platelet aggregation,18%by small red blood cells,and the remaining 70%were caused by large platelets;Among 50 patient specimens in the experimental group,14%were caused by small red blood cells,and 86%were caused by large platelets.When the platelet histogram was normal,there was no significant difference among the results from the three methods(P>0.05);When the platelet histogram was abnormal,there were differences between any two of the three methods(P<0.05).Conclusion When the platelet histogram are abnormal,we need to review them and communicate with the clinician if necessary.When the platelet histogram are abnormal,we can use PLT-O to detect them.
