赤峰市牧区蒙古族居民基层医疗卫生服务利用现状及影响因素分析

摘　　要：目的了解赤峰市牧区蒙古族居民基层医疗卫生服务利用现状,并分析影响居民基层医疗卫生服务利用的因素。方法按照赤峰市牧区经济水平由高到低,抽取克什克腾旗、阿鲁科尔沁旗、巴林右旗3个牧区旗县,每个旗县随机抽取3个苏木;再从这3个苏木中抽取牧区蒙古族居民比较密集的嘎查;然后采用方便抽样方法共抽取研究对象1164名,有效研究对象987名,有效率为85%;采用自行设计的调查问卷进行面对面调查,在当地卫生室、乡镇卫生院及社区卫生服务中心集中调查和入户调查两种方式进行调查;采用描述性分析,卡方检验、多元Logistics回归分析进行统计描述。结果居民整体基层医疗卫生服务利用率为79.23%;经多因素Logistics回归分析结果显示,不同年龄、慢性病、体检频率及慢性病随访服务均是基层医疗卫生服务利用的影响因素;在保健知识获取途径中,健康讲座、医生和微信等多媒体渠道是基层医疗卫生服务利用的影响因素。结论居民基层医疗卫生服务利用情况直接反应基层医疗卫生机构整体建设和医疗水平,通过加强基层卫生人员服务能力,并不断引进和培养全科医生队伍,深化家庭医生签约服务,开展特色医疗,为蒙古族居民提供优质长期的医疗卫生服务。Objective To know about the current situation of the utilization of primary healthcare services for Mongolian residents in the pastoral areas of Chifeng City,and to analyze the factors affecting the utilization of primary healthcare services.Methods The three pastoral areas,Kashkent Banner,Aru Kerqin Banner and Bahrain Right Banner,were selected according to the economic levels of Chifeng City pastoral areas.From each of these Banners 3 Sumu(an administrative division)were chosen;and then from these three Sumu gacha(administrative village),where there were densely populated Mongolian residents,were chosen.Totally 1,164 subjects were selected by convenient sampling method,and 987 valid subjects were studied,with an effective rate of 85%.Face-to-face surveys were conducted using self-designed questionnaires,including centralized surveys in local clinics,township hospitals and community health service centers and household surveys.Descriptive analysis,Chi-square test,and multiple logistics regression analysis were used for statistical description.Results The utilization rate of primary healthcare services for residents was 79.23%.Multivariate logistic regression analysis results showed that age,chronic diseases,frequency of physical examinations,and follow-up services for chronic diseases were the influence factors of the utilization of primary healthcare services.Among approaches of acquiring health knowledge,multimedia such as health lectures,doctors and WeChat were the influence factors.Conclusions Residents’utilization of primary healthcare services reflects directly the comprehensive construction and healthcare capacity of primary healthcare institutions.Mongolian residents are provided with long-term and high-quality healthcare services by strengthening the service capacity of health personnel in grassroots healthcare institutions,continuously introducing and cultivating general practitioners,enhancing contracted family doctor services and carrying out medical treatment with characteristics.
