腔内同轴技术弹簧圈栓塞术治疗脾动脉瘤13例

摘　　要：目的分析腔内同轴技术弹簧圈栓塞术治疗脾动脉瘤的临床价值和疗效。方法回顾性分析2017年12月至2020年1月上海交通大学医学院附属第九人民医院采用腔内同轴技术行弹簧圈栓塞术治疗的13例脾动脉瘤患者的临床资料及治疗结果。结果13例患者手术均成功,技术成功率100%。平均手术时间(39.1±11.9)min,平均辐射剂量(423.0±188.3)mGy。术后1例患者出现造影剂过敏反应,其余均无发热、腹痛等,术后平均住院(2.6±1.2)d。平均随访(9.5±5.8)个月,患者无死亡,未见动脉瘤复发、弹簧圈移位、异位栓塞、脾梗死等并发症。结论腔内同轴技术弹簧圈栓塞术治疗脾动脉瘤安全、有效,短期随访效果满意,具有良好的临床应用推广前景。Objective To evaluate the clinical value and effect of coil embolization with endovascular telescoping technique in the treatment of splenic artery aneurysm.Methods The clinical data and treatment outcome of 13 patients with splenic aneurysm treated by coil embolization with endovascular telescoping technique in Shanghai Ninth People’s Hospital Affiliated to Shanghai Jiao Tong University from December 2017 to January 2020 were analyzed retrospectively.Results The operation was successful in all patients,and the technical success rate was 100%.The average operation time was(39.1±11.9)min,and the average radiation dose was(423.0±188.3)mGy.One patient had an allergic reaction to the contrast medium after the operation,and none of the others had fever or abdominal pain.The average postoperative hospital stay was(2.6±1.2)days.The average follow-up was(9.5±5.8)months.There was no death,and no complications such as recurrence of aneurysm,coil displacement,ectopic embolism,splenic infarction.Conclusion Coil embolization with endovascular telescoping technique is safe and effective for the treatment of splenic artery aneurysm.Its short-term follow-up results are satisfactory and has a good prospect of clinical application and promotion.
