从脾论治功能性腹泻机制探析

摘　　要：功能性腹泻(FDr)是临床常见的消化系统疾病,发病率高,发病机制尚不完全明确,现代医学缺乏行之有效的治疗方法。文章从中医治疗出发,基于从脾论治理论分别从脾胃虚弱,运化失职、肝气郁结,脾气不升、肾阳虚衰,脾阳不振3个方面对该病病机进行探讨。中医治疗功能性腹泻脾虚证以运脾化湿为基本治疗原则,佐以疏肝、温肾之法,使脏腑之间互相协调,达到既互相制约又互相促进的动态平衡关系,恢复其正常的生理功能,从根本上改善患者的症状。Functional diarrhea(FDr)is a common digestive system disease in clinic.Its incidence rate is high,the pathogenesis is not completely clear and modern medicine lacks effective treatment methods.Starting from the treatment of traditional Chinese medicine,based on the theory of spleen treatment,this paper discusses the pathogenesis of this disease from three aspects,namely the deficiency of spleen and stomach and the disorder of transportation,liver qi stagnation and spleen qi is not ascending,kidney yang deficiency and spleen yang devitalization.In the treatment of functional diarrhea with the syndrome of spleen deficiency from traditional Chinese medicine,the basic principle is transporting spleen for eliminating dampness,accompanied by the method of dispersing stagnated liver qi and warming the kidney,so that the zang-fu viscera can coordinate with each other to achieve a dynamic equilibrium relationship of mutually restriction and promotion,restore its normal physiological function,and fundamentally improve the symptoms of patients.
