小金丹联合逍遥丸对不同临床病理进展时期乳腺癌患者中医证候变化的影响

摘　　要：目的:考察小金丸联合逍遥丸对于不同临床病理进展时期乳腺癌患者的中医证候变化的影响,评价药物干预前后气血阴阳,邪正盛衰变化情况。方法:采用前后对照原则,选取2016年6月至2018年12月于山东中医药大学附属医院肿瘤科收治并经病理确诊的的乳腺癌患者89例,以TNM分期为临床病理分期标准,根据中医证素评价标准进行量化评分,及相关中医证候评价标准,判断干预前后,不同病理进展期的中医证候变化情况。结果:干预前Ⅰ期患者实证证素积分更高,Ⅲ、Ⅳ期患者虚证证素积分更高,Ⅱ期患者实证和虚证证素均有分布;干预后,证素方面,Ⅰ期患者气郁证证素明显减少,Ⅱ期气虚证、阳虚证、痰湿证和血瘀证明显减少,Ⅲ期气虚证、血虚证、阳虚证、阴虚证和血瘀证明显减少(P<0.05)。证候方面,Ⅰ期肝郁痰凝证患者明显减少,Ⅱ期肝郁痰凝和冲任失调者明显减少,Ⅲ期肝郁痰凝和正虚毒炽者明显减少,Ⅳ期正虚毒炽者明显减少(P<0.05)。结论:小金丹联合逍遥丸可针对乳腺癌不同临床病理时期,通过改善乳腺癌患者的证候体质达到治疗的目的。Objective:To investigate the effects of Xiaojin Pills combined with Xiaoyao Powder on TCM syndrome changes of breast cancer at different stages of clinicopathological progression and to evaluate the changes of qi,blood,yin and yang before and after drug intervention in patients with breast cancer.Methods:Eighty-nine patients with breast cancer who were admitted to department of oncology,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2016 to December 2018 were selected according to the principle of pre-and post-contrast.The TNM stage was used as the clinical pathological staging standard,and the quantitative score was made according to the evaluation standard of TCM syndromes and related TCM syndrome evaluation criteria,to judge the TCM syndrome changes of different pathological progression before and after intervention.Results:Before the intervention,the patients in stageⅠwere mainly excess syndrome,the patients in stageⅢandⅣwere mainly deficiency syndrome,and the patients in stageⅡwere distributed in both excess syndrome and deficiency syndrome.After intervention,there was significant improvement in qi stagnation syndrome in stageⅠpatients,qi deficiency syndrome,yang deficiency syndrome,phlegm dampness syndrome and blood stasis syndrome in stageⅡpatients,qi deficiency syndrome,blood deficiency syndrome,yang deficiency syndrome,yin deficiency syndrome and blood stasis syndrome in stageⅢ(P<0.05).In terms of syndrome,the numbers of patients with liver depression and phlegm coagulation syndrome in stageⅠ,liver depression and phlegm coagulation syndrome and thoroughfare-controlling vessels disharmony in stageⅡ,liver depression and phlegm coagulation syndrome and healthy qi deficiency and blazing toxin in stageⅢ,healthy qi deficiency and blazing toxin in stageⅣwere decreased significantly(P<0.05).Conclusion:Xiaojin Pills combined with Xiaoyao Powder can be used to improve the syndrome constitution of breast cancer patients in different clinical and pathologic
