脑络通合剂痰瘀同治治疗慢性阻塞性肺疾病合并抑郁状态临床观察

摘　　要：目的:探讨脑络通合剂痰瘀同治治疗慢性阻塞性肺疾病(COPD)合并抑郁状态的临床疗效观察。方法:将84例痰瘀阻滞型COPD合并抑郁状态的患者随机分为实验组和对照组各42例,两组在对COPD的治疗方案完全一致的基础上,对照组给予西药草酸艾斯西酞普兰片治疗,实验组给予中药制剂脑络通合剂治疗,治疗6周后比较两组症候、肺功能指标及量表评分。结果:治疗后在临床疗效、部分肺功能指标(FEV1、FEV1%)上,实验组改善均优于对照组(P<0.05);实验组各量表评分均较治疗前显著改善(P<0.01,P<0.05);治疗后实验组mMRC、CAT量表评分显著优于对照组(P<0.05,P<0.01)。对照组在HAMA、HAMD评分上较治疗前均有改善(P<0.05,P<0.01)。结论:脑络通合剂痰瘀同治治疗COPD合并抑郁状态疗效良好,可以减轻症状,改善肺功能,且服用安全,值得进一步研究推广。Objective:To explore the clinical effect of Naoluotong Mixture in treating chronic obstructive pulmonary disease(COPD)with depression.Methods:Eighty-four cases of COPD with depression were randomly divided into the experimental group and the control group,42 cases in each group.Based on completely consistent treatment plan for COPD,the control group was treated with Escitalopram Oxalate Tablets,and the experimental group was treated with Naoluotong Mixture.After 6 weeks of treatment,the symptoms,lung function indexes and scale scores of the two groups were compared.Results:After the treatment,the experimental group was superior to the control group in terms of clinical efficacy and partial lung function index(P<0.05).Compared with the scale score before treatment,the experimental group was different in terms of scale score(P<0.01,P<0.05).The experimental group’s mMRC,CAT scale scores were significantly better than the control group’s(P<0.05,P<0.01).After the treatment,the control group’s HAMA,HAMD were significantly better(P<0.05,P<0.01).Conclusion:Naoluotong Mixture is effective in the treatment of COPD with depression.It can reduce symptoms,improve lung function,and is safe to take.It is worthy of further study and promotion.
