宫腔灌注hCG治疗不同年龄阶段反复种植失败患者的研究

摘　　要：目的探讨人绒毛膜促性腺激素(hCG)宫腔灌注对反复种植失败患者妊娠结局的影响。方法选择在我院生殖医学科行ART治疗反复移植失败(RIF)的准备再次行冻融胚胎移植的患者72例,移植前的一个月经周期,监测排卵后连续3天行宫腔灌注。随访妊娠结局,进行年龄分组比较分析,不孕原因分组比较分析。结果RIF患者经过3次宫腔灌注后行FET妊娠率达到58.33%,其中临床妊娠率为47.22%,生化妊娠率为11.11%。宫腔灌注hCG在≤35岁RIF患者及>35岁RIF患者的妊娠结局是有统计学差异(P=0.00),随着年龄的增大妊娠率越低。宫腔灌注hCG在不同原因不孕的RIF患者的妊娠结局是无统计学差异(P=0.396)。结论宫腔灌注hCG能有效提高RIF患者的妊娠率。于≤35岁RIF患者可以考虑进行宫腔灌注以提高妊娠率。对于>35岁患者如何提高妊娠率需进一步探讨。Objective To explore the effect of intrauterine administration with hCG on pregnancy outcomes in patients with recurrent implantation failure(RIF).Methods A total of 72 RIF patients received frozenthawed embryo transfer(FET)were enrolled.At menstrual cycle before FET,Those RIF patients were intrauterined administration with hCG three days in a row after ovulation.Those pregnancy outcomes were collected.Then comparatived analysis between≤35 years group and>35 years group,and among infertility of different causes.Results The FET pregnancy ran achieve 58.33%at the RIF patients,who were intrauterined administration with hCG three days in a row after ovulation.Clinical pregnancy rate was 47.22%.Biochemical pregnancy rate was 11.11%.There were significant differences in the clinical pregnancy rate between≤35 years group and>35 years group.The clinical pregnancy rate in the≤35 years group were significantly higher than those in the>35 years group(P=0.00).There were no significant differences in infertility of different causes(P=0.396).Conclusions The pregnancy rate of RIF patients in≤35 years can effective improve,who intrauterined administration with hCG three days in a row after ovulation.While The Method to improve the pregnancy rate of RIF patients in>35 years should be further discussion.
