艾儒棣基于“内外并治”分型辨治湿疹经验

摘　　要：介绍艾儒棣教授基于"内外并治"理论运用中药药浴疗法辨治湿疹经验。在治疗中,艾儒棣主张"内外并治"。根据病程分阶段整体辨证结合外用中药药浴辨治湿疹可获良效。内治方面,具体辨治可分为湿热蕴肤证,治以清热祛湿止痒,方选湿疹1号方(马齿苋汤合简化消风散加减);脾虚湿蕴证,治以健脾祛湿止痒,方选湿疹2号方(异功散加减);血虚风燥证,治以祛风养血止痒,方选湿疹3号方(当归饮子加减)。外治方面,湿疹急性期中药药浴重在清热凉血、祛风止痒;湿疹亚急性期重在清热解毒、祛湿止痒;湿疹慢性期重在养血滋阴、润燥止痒。Based on the theory of‘principle of combined internal and external treatment’,AI Ru-di uses traditional chinese medicine bath therapy to treat eczema.In his treatment,AI Ru-di advocated‘principle of combined internal and external treatment’.According to the disease course,the overall syndrome differentiation in stages combined with external use of traditional Chinese medicine bath can get good effect.Internally,specific differentiation and treatment include damp and hot accumulate skin syndrome,which is treated by clearing heat and expelling damp and relieving itching,and treated by Machixian Decoction and Xiaofeng Powder,spleen deficiency dampness syndrome,which is treated by tonifying spleen and clearing damp and relieving itching,and treated by Yigong Powder,blood deficiency and wind dryness syndrome,which is treated by dispelling wind and nourishing blood and relieving itching,and treated by Danggui Yinzi Decoction.Externally,Chinese medicine bath in the acute stage of eczema is mainly to clear heat and cool blood,dispe wind and relieve itching.In the subacute stage of eczema,detoxification and antipruritic effect of clearing damp are emphasized.Chronic eczema emphasizes nourshing dryness and relieving itching.
