摘 要:目的:研究清温并用法对慢加急性肝衰竭大鼠肝组织Treg/Th17型细胞因子及转录因子FoxP3 mRNA、ROR-γt mRNA的影响。方法:将130只大鼠随机分为正常组10只,造模组120只,造模组采用牛血清白蛋白致敏建立免疫性肝纤维化大鼠模型,然后以D-氨基半乳糖+脂多糖攻击建立ACLF大鼠模型共81只,随机分为模型组20只,清法组20只,温法组20只,清温并用法组21只。动态观察不同时间节点(1、12、24h)各组肝功能(ALT、TBIL)和肝组织Treg/Th17型细胞因子(IL-10、TGF-β、IL-17A、IL-23、TNF-α)转录因子(FoxP3、ROR-γt)mRNA表达水平的变化。结果:①与正常组比较,模型组大鼠ALT、TBIL显著升高(P<0.01),肝组织细胞因子IL-17A、IL-23、TNF-α、IL-10、TGF-β及转录因子ROR-γt mRNA、FoxP3 mRNA表达水平亦显著升高(P<0.01);②与模型组比较,清法组、温法组及清温并用法组ALT、TBIL水平均显著降低(P<0.05),Treg/Th17型细胞因子、IL-17A、IL-23、TNF-α、IL-10、TGF-β及转录因子ROR-γt mRNA、FoxP3 mRNA表达水平均显著降低(P<0.05),其中清温并用法组较清法组、温法组降低更显著(P<0.05)。结论:清温并用法抗肝衰竭的作用机制可能与抑制肝脏转录因子FoxP3及ROR-γt的表达,降低Treg/Th17细胞分泌细胞因子水平有关。Objective:To observe the effects of clearing and warming method on the expression of Treg/Th17 related transcription factors FoxP3 mRNA and ROR-γt mRNA in rats with acute-on-chronic liver failure(ACLF).Methods:A total of 130 rats were randomly divided into normal group(10 rats),the model group(120 rats).Immune liver fibrosis rats model was established by sensitization of bovine serum albumin,and then ACLF model was established by intraperitoneal injection of D-galactosamine+Lipopolysaccharide solution.The 81 ACLF model rats were randomly divided into model group(20 rats),the clearing method group(20 rats),the warming method group(20 rats)and the clearing and warming method group(21 rats).Changes in Treg/Th17 related cytokines(IL-10,TGF-β,IL-17 A,IL-23,TNF-α),and transcription factors(FoxP3 mRNA,ROR-γt mRNA)were dynamically observed in all groups within 24 hours(1 h,12 h,24 h).Results:①Alanine aminotransferase(ALT)and total bilirubin(TBIL)levels in the model group were significantly higher than those in the normal group(P<0.01),cytokine IL-10,TGF-β,IL-17 A,IL-23,TNF-αand transcription factors FoxP3 mRNA,ROR-γt mRNA in liver were significantly increased(P<0.01).②Compared with model group,ALT and TBIL levels in the clearing group,warming group and the clearing and warming group were declined(P<0.05),Treg/Th17 related cytokines and transcription factors were also descended(P<0.05).The clearing and warming method group was better at declining cytokines and transcription factors than the clearing group and warming group(P<0.05).Conclusion:The anti-liver failure mechanism of clearing and warming method may be related to the inhibition of the expression of liver and transcription factors FoxP3,ROR-γt and decreasing the secretion of Treg/Th17 related cytokines.
