摘 要:目的探讨布地奈德、沙丁胺醇、异丙托溴铵联合雾化吸入应用于小儿支气管肺炎治疗的临床效果。方法选取2017年1月至2019年12月贵州省人民医院白云分院收治的支气管肺炎患儿90例,按照随机数字表法分为常规组和试验组,每组45例。常规组患儿接受常规+布地奈德雾化吸入治疗,试验组患儿接受常规+布地奈德、沙丁胺醇、异丙托溴铵联合雾化吸入治疗。对比两组患儿治疗前后的发热消退时间、喘息和咳嗽缓解时间及肺部啰音消失时间,以及血清炎症指标水平。结果试验组患儿的退热时间、喘息及咳嗽的缓解时间、肺部啰音消失时间均较常规组更早(P<0.05);治疗后的超敏C反应蛋白(hs-CRP)、白细胞(WBC)计数、红细胞沉降率(ESR)水平均较常规组更低(P<0.05);临床总有效率、生活质量量表(SF-36)评分均较常规组均更高(P<0.05);不良反应发生率与常规组比较差异无统计学意义(P>0.05)。结论布地奈德、沙丁胺醇、异丙托溴铵联合雾化吸入治疗小儿支气管肺炎,可进一步改善患儿的临床症状和体征,且不良反应较少。Objective To explore the efficacy of budesonide,salbutamol,ipratropium bromide combined with inhalation in the clinical treatment of bronchial pneumonia in children.Methods A total of 90 children with bronchial pneumonia in Baiyun Branch of Guizhou Provincial People's Hospital from January 2017 to December 2019 were selected and divided into the conventional group and the experimental group according to the random number method,with 45 patients in each group.The children in the conventional group received conventional+budesonide atomization inhalation,while the children in the experimental group received conventional+budesonide,salbutamol,and ipratropium combined atomization inhalation.The time for fever resolution,wheezing and cough relief,rhonchus in the lungs,and serum inflammatory markers were compared between the two groups before and after treatments.Results The time of reducing fever,relieving wheezing and coughing,and disappearing time of rhonchus in lung were earlier in the experimental group than in the conventional group(P<0.05).After treatment,the number of hypersensitive C-reactive protein(HS-CRP),white blood cell(WBC)and erythrocyte sedimentation rate(ESR)were all lower than those in the conventional group(P<0.05).Overall clinical efficiency and sF-36 scores were higher than those of the conventional group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the conventional group and the conventional group(P>0.05).Conclusion The combination of budesonide,salbutamol,ipratropium bromide and inhalation in the treatment of pediatric bronchopneumonia can further improve the clinical efficacy of children with fewer adverse reactions.
