超声引导在乳晕弧形切口乳腺纤维瘤切除术中的应用

摘　　要：目的:探讨应用超声引导乳晕边缘弧形切口手术治疗乳腺纤维瘤的临床效果。方法:选择2017年1月~2019年6月我院治疗的乳腺纤维瘤患者96例,随机分为观察组与对照组A,观察组49例(68个瘤),对照组A47例(66个瘤),另外收集2017年前采用传统放射状切口治疗的乳腺纤维瘤患者50例(70个瘤)作为对照组B。观察组采用超声引导乳晕边缘弧形切口手术治疗,对照组A选择乳晕边缘切口手术治疗,对照组B采用传统放射状切口手术治疗,比较三组手术漏切误切情况、手术时间及患者满意度。结果:手术时间平均为观察组(32.48±6.18)min,对照组A(41.44±8.70)min,对照组B(36.30±7.09)min,观察组手术时间明显短于对照组A和B,差异有统计学意义(P<0.05);观察组漏切误切率0%;对照组A为7.58%;对照组B为8.57%,观察组漏切误切率明显低于两个对照组,差异有统计学意义(P<0.05)。观察组满意率为100%,明显高于对照组A为89.36%和对照组B为74.0%,差异有统计学意义(P<0.05)。结论:应用超声引导乳晕边缘弧形切口手术治疗乳腺纤维瘤效果显著,快捷准确,避免漏误切情况,术后无明显疤痕,患者治疗满意度高。Purpose:To investigate the clinical effect of ultrasonic guidance in resection operation of areola curved incision breast fibroma.Method:96 patients with breast fibroma treated in our hospital from January 2017 to June 2019 were selected.The patients were divided into observation group and control group A randomly,with 49 cases(68 tumors)in observation group and 47 cases(66 tumors)in control group A.In addition,50 cases(70 tumors)of breast fibroma treated with conventional radial incision before 2017 were collected as control group B.The observation group was treated with ultrasonically guided curved incision at the edge of the areola,while the control group A was chosen incision at the edge of the areola,and the control group B was treated with traditional radial incision.The results of missed operation,operation time and patient satisfaction were compared.Results:The average operation time was 32.48±6.18 min in the observation group,41.44±8.70 min in the control group A,and 36.30±7.09 min in the control group B.The operation time in the observation group was significantly shorter than that in the control group,with significantly statistical difference(P<0.05).The rate of missed cut was 0%in the observation group,while it was 7.58%in control group A and 8.57%in control group B.The rate of missed cut was significantly lower in the observation group than in two control groups,with significantly statistical difference(P<0.05).The satisfaction rate of the observation group was 100%,and it was higher than 89.36%in control group A and 74.0%in control group B,with significantly statistical difference(P<0.05).Conclusion:The application of ultrasound guidance curved incision in the treatment of breast fibroma was effective,fast and accurate,avoiding missed incision,with no obvious postoperative scar,and high satisfaction.
