血清CRP、CKBB和MMP-2联合检测对轻型创伤性脑损伤危险分层的价值

摘　　要：目的探讨血清C反应蛋白(CRP)、脑型肌酸激酶同工酶(CKBB)和基质金属蛋白酶-2(MMP-2)联合检测在预测轻型创伤性脑损伤(MTBI)患者CT扫描结果阳性或阴性中的作用,以期对MTBI进行危险分层。方法选择2016年6月至2019年11月该院收治的92例MTBI患者。根据CT结果分为观察组和对照组,伤后4 h内采集标本,采用酶联免疫吸附试验法检测血清CKBB和MMP-2水平,采用免疫比浊法测定血清CRP水平。应用受试者工作特征(ROC)曲线分析CRP、CKBB和MMP-2联合检测区分MTBI患者CT扫描结果阳性与阴性的能力。结果92例MTBI患者中CT扫描结果阴性79例,阳性13例。观察组血清CRP、CKBB和MMP-2水平显著高于对照组(P<0.05)。CRP、CKBB和MMP-2联合检测有较好区分MTBI患者CT扫描结果阳性与阴性的能力,其曲线下面积为0.945(95%CI 0.882~0.999)。结论血清CRP、CKBB和MMP-2联合检测可以为影像学检查提供有价值的参考,减少不必要的CT扫描带来的辐射。Objective To investigate the effect of the combined detection of C reactive protein(CRP),brain-type creatine kinase isoenzyme(CKBB)and matrix metalloproteinase-2(MMP-2)for predicting the CT scanning results positive or negative in mild traumatic brain injury(MTBI)in order to conduct the risk stratification in MTBI.Methods Ninety-two cases of MTBI in this hospital from June 2016 to November 2019 were selected and divided into the observation group and control group according to the CT results.The sample was collected at 4 h after injury.The serum CKBB and MMP-2 levels were measured by ELISA,the CRP level was measured by adopting the immunoturbidimetry.The receiver operating characteristic(ROC)curve was used to analyze the ability of combined detection of CRP,CKBB and MMP-2 for differentiating CT scanning results positive and negative in MTBI patients.Results Among 92 cases of MTBI,there were 79 cases of CT scanning results negative and 13 cases of CT scanning results positive.The levels of CRP,CKBB and MMP-2 in the observation group were significantly higher than those in the control group(P<0.05).The combined detection of CRP,CKBB and MMP-2 had good ability for differentiating CT scanning results positive and negative in the patients with MTBI.The area under the ROC curve was 0.945(95%CI 0.882-0.999).Conclusion The combined detection of serum CRP,CKBB and MMP-2 can provide the valuable reference for the imageological examination and reduce the radiation bringing by unnecessary CT scanning.
