胃大部分切除术后胃空肠结肠瘘1例并文献复习

摘　　要：目的探讨行胃大部切除术后出现胃空肠结肠瘘病因、临床症状、诊断与治疗方法。方法分析收治的1例行胃大部切除术后出现胃空肠结肠瘘患者的治疗过程,后期随访,同时结合文献复习。结果患者行手术治疗,术后6个月随访,患者无腹泻复发,BMI指数由术前17.1恢复到19.8。结论胃大部分切除术后出现胃空肠结肠瘘非常罕见,常见原因是吻合口溃疡,主要检查方法为钡剂灌肠联合结肠镜检查,外科手术是最佳治疗方法。Objective To investigate the etiology,clinical symptoms,diagnosis,and treatment of gastrojejunocolic fistula after subtotal gastrectomy.Methods The treatment course of a patient with gastrojejunocolic fistula after subtotal gastrectomy in our hospital was analyzed,followed up later,and the literature review was combined.Results The patient underwent surgical treatment and was followed up 6 months after the operation.There was no recurrence of diarrhea in the patient,and the BMI improved from 17.1 to 19.8.Conclusion Gastrojejunocolic fistula after subtotal gastrectomy is very rare.The common cause is an anastomotic ulcer.The main examination method is barium enema combined with colonoscopy.Surgery is the best treatment.
