微波消融联合培土消瘿方治疗结节性甲状腺肿疗效观察

摘　　要：目的:观察微波消融联合培土消瘿方治疗结节性甲状腺肿的临床疗效,探讨中医药干预治疗的优势。方法:收集60例结节性甲状腺肿患者随机分为对照组和试验组各30例,接受甲状腺微波消融术。对照组术后口服左甲状腺素片,试验组给予口服扶正消瘿汤共治疗3个月,并分析2组临床疗效、安全性和术后复发率。结果:治疗组在总有效率及症状积分上优于对照组,术后颈部疼痛、结节直径及体积缩小程度试验组优于对照组,试验组转化生长因子-α(TGF-α)、胰岛素样生长因子-1(IGF-1)较对照组下降明显,转化生长因子β1(TGF-β1)上升明显,对照组前后改变无统计学意义;试验组对甲状腺功能无明显改变,而对照组促甲状腺激素(TSH)治疗后明显下降,不良反应发生率、12个月内复发率试验组低于对照组。结论:结节性甲状腺肿微波消融术后口服培土消瘿方效果显著,安全有效,复发率低。Objective:To observe the clinical effect of microwave ablation combined with Peitu Xiaoying Formular in the treatment of nodular goiter,and discuss the efficacy of TCM intervention in the treatment.Methods:A total 60 cases with nodular goiter were observed,and randomly divided into control group and experimental group,30 cases in each group.Control group was treated by Levothyroxine Sodium Tablets(LST)and experimental group was treated with Peitu Xiaoying Formular in the basis of microwave ablation therapy.Three months were needed for one course.Clinical efficacy,safety and recurrence rate of the two groups were recorded and analyzed.Results:The total effective rate and symptom score of the experimental group were better than control group.The relief degree of neck pain,the reduction of the maximum nodule diameter and thyroid volume in the experimental group was superior to the control group.The transforming growth factorα(TGF-α),insulin-like growth factor-1(IGF-1)in the experimental group down significantly compared with the control group,and transforming growth factorβ1(TGF-β1)increased significantly.While there was no significant change in the control group.There was no significant change in thyroid function of experimental group,while thyroid stimulating hormone was significantly decreased in the control group after treatment.The incidence of adverse reactions and recurrence rate within 12 months in the treatment group was lower than the control group.Conclusion:After microwave ablation of nodular goiter,treat with Peitu Xiaoying Formular has obvious effect,safety,low recurrence rate.
