黄芪治疗肺脾气虚型直肠内脱垂的疗效及机制研究

摘　　要：目的:比较不同黄芪剂量的补中益气汤对肺脾气虚型直肠内脱垂患者的疗效及对直肠组织压力感受器蛋白Piezo 2表达的影响。方法:随机抽取10例行PPH手术的混合痔患者作为空白对照组,将30例肺脾气虚型直肠内脱垂患者随机分为实验组1(无中药治疗+PPH)、实验组2(30 g黄芪的补中益气汤+PPH)和实验组3(60 g黄芪的补中益气汤+PPH)。评价补中益气汤治疗前后患者肛门坠胀和Longo评分,并采用免疫蛋白印迹法(Western blot)检测直肠组织压力感受器蛋白Piezo 2的表达。结果:补中益气汤干预后患者肛门坠胀、Longo评分较服药前均有明显改善,手术干预后患者Longo评分、肛门坠胀症状评分较术前均有明显改善,随访各实验组术后1年Longo评分较术后半年均升高,与空白对照组比较实验组1的Piezo 2表达明显升高。结论:不同剂量黄芪的补中益气汤可改善直肠内脱垂症状,从而影响直肠组织压力感受器蛋白Piezo2表达程度,术前使用补中益气汤可能对直肠内脱垂术后疗效有促进作用。Objective:To compare the therapeutic effects of Buzhong Yiqi Decoction(BYD)with different doses of Radix Astragali on internal rectal prolapse patients with Qi deficiency of lung and spleen and the expression of Piezo 2 in rectal tissues and to evaluate its possible mechanism.Methods:Ten volunteers who underwent hemorrhoids were randomly selected as the control group.Thirty patients were randomly divided into group1(without BYD+PPH),group2(BYD with 30 g Radix Astragali was given before surgery+PPH)and group3(BYD with 60 g Radix Astragali was given before surgery+PPH).The outcomes were evaluated by the scores of anal pendant expansion and Longo ODS score,and the expression level of Piezo 2 in rectal tissues was detected by Western blot method.Results:After intervention with BYD,the outcomes of the score of anal pendant expansion and Longo ODS score were improved.And the Longo ODS score and the symptom of anal pendant expansion were significantly improved after surgery.Compared with the outcomes of Longo ODS score at 6 months after surgery,it became worse at one year after surgery.The expression level of Piezo 2 in group 1 was higher than that in control group.Conclusion:The preoperative use of BYD may have a positive effect on long-term postoperative outcomes.BYD with different doses of Radix Astragali can improve the preoperative symptoms in patients with internal rectal prolapse and lead to the different expression level of Piezo 2 in rectal tissues.
