柴胡疏肝汤联合重复经颅磁刺激治疗围绝经期抑郁观察

摘　　要：目的:探讨柴胡疏肝汤联合重复经颅磁刺激治疗对围绝经期抑郁患者神经内分泌功能影响。方法:选取2016年1月至2018年12月十堰市中医医院收治的围绝经期抑郁患者120例作为研究对象,按照入院顺序分为对照组和观察组,每组60例。2组予盐酸文拉法辛缓释片治疗,观察组再予以柴胡疏肝汤联合重复经颅磁刺激治疗。观察2组入组时、疗程完成时性激素、内分泌、炎性反应因子、汉密尔顿抑郁量表(HAMD)、改良Kupperman评分(KI评分)、抑郁自评量表(SDS)、匹斯堡睡眠质量指数(PSQI)比较,并比较评价疗程完成时疗效。结果:疗程完成时,2组患者5-HT、NE、DA、Glu、Asp、E2均较入组时均显著升高,GABA、Gly、FSH、LH、HAMD、KI评分、SDS、PSQI、IL-1β、IL-2、IL-6、CRP均较入组时显著下降,且观察组上述指标改善幅度高于对照组患者(P<0.05),完成治疗后观察组患者总有效率高于对照组患者(P<0.05)。结论:柴胡疏肝汤联合重复经颅磁刺激可改善围绝经期抑郁患者性激素、神经内分泌水平,抑制炎性反应,从而改善症状,提高疗效。Objective:To explore the effects of Chaihu Shugan Decoction combined with repetitive transcranial magnetic stimulation on neuroendocrine function in patients with perimenopausal depression.Methods:A total of 120 women with perimenopausal depression in Shiyan City Chinese Medicine Hospital were divided into a control group and a study group,with 60 cases in each group.The 2 groups were treated with venlafaxine hydrochloride sustained-release tablets,and the study group was treated with Chaihu Shugan Decoction combined with repeated transcranial magnetic stimulation.The sex hormone,endocrine,inflammatory factors,Hamilton Depression Scale(HAMD),modified Kupperman score(KI score),self rating Depression Scale(SDS)and Pittsburgh sleep quality index(PSQI)were observed and compared when the 2 groups were enrolled and at the end of the course of treatment.Results:At the end of the course of treatment,5-HT,NE,DA,Glu,ASP,E2 in the 2 groups were significantly higher than those in the control group,and GABA,Gly,FSH,LH,HAMD,Ki scores,SDS,PSQI,IL-1β,IL-2,IL-6,CRP in the study group were significantly lower than those in the control group(P<0.05).Conclusion:ChaiHu Shugan Decoction combined with repetitive transcranial magnetic stimulation can improve the level of sexual hormone and neuroendocrine,inhibit the inflammatory reaction,improve the symptoms and the curative effects.
