肿瘤科护士生命意义感现状及影响因素分析

摘　　要：[目的]调查肿瘤科护士生命意义感现状,并分析其影响因素。[方法]采用一般资料调查表、生命意义感量表、姑息护理知识量表、死亡态度量表、共情能力量表、照顾临终病人量表对528名肿瘤科护士进行调查,并进行肿瘤科护士姑息护理知识、死亡态度、共情能力与生命意义感相关性分析及影响因素分析。[结果]肿瘤科护士生命意义感总分为(46.91±8.42)分,姑息护理知识量表总分为(10.66±1.78)分,死亡态度量表中死亡恐惧维度得分为(21.45±4.87)分、死亡逃避维度得分为(15.63±3.86)分、自然接受维度为(19.22±3.08)分、逃离接受维度为(14.28±3.84)、趋近接受维度得分为(30.04±6.61)分,共情能力量表总分为(79.56±4.59)分,照顾临终病人态度量表总分为(104.07±10.66)分;姑息护理知识总分、死亡恐惧得分、死亡逃避得分、自然接受得分、趋近接受得分、照顾临终病人态度总分与生命意义感总分呈正相关(P<0.01);整体照顾意识、自然接受、死亡逃避、心理社会精神支持、姑息护理哲理和原则、积极态度、基础学历、讨论死亡话题的情景、逃离接受、趋近接受是肿瘤科护士生命意义感的影响因素。[结论]肿瘤科护士生命意义感处于中等水平,应针对生命意义感影响因素,提升个体幸福感,并引导肿瘤病人思考生命意义。Objective:To investigate the status quo of nurses’sense of life in the oncology department and analyze its influencing factors.Methods:A total of 528 oncology nurses conducted the survey with The general information questionnaire,The Meaning in Life Questionnaire(MLQ),Palliative Care Quiz for Nursing(PCQN),Death Attitude Profile-Revised(DAP-R),the Jefferson Scale of Empathy-Health Professionals(JSE-HP),and the Nursing for Dying Patient Scale.The correlated analysis and influencing factors analysis of nurses’sense of life in the oncology department was conducted.Results:The MLQ total score of oncology nurses was(46.91±8.42)points,the PCQN total score was(10.66±1.78)points,and the death fear dimension score of DAP-R was(21.45±4.87)points,death avoidance dimension score was(15.63±3.86)points,natural acceptance dimension was(19.22±3.08)points,escape acceptance dimension was(14.28±3.84)points,approaching acceptance dimension score was(30.04±6.61)points.The JSE-HP total score was(79.56±4.59)points,and the total score of the Nursing for Dying Patient Scale was(104.07±10.66)points.the total score of PCQN,DAP-R,death avoidance score,natural acceptance score,approach acceptance score,and the total score of nursing for dying patients were positively correlated with the MLQ total score(P<0.01).The overall caring awareness,natural acceptance,death avoidance,psychosocial support,philosophy and principles,positive attitude,basic education,discussion of death topics Circumstances,fleeing acceptance,and approaching acceptance were the influencing factors of oncology nurses’sense of life.Conclusions:Oncology nurses had an intermediate sense of meaning in life.The oncology nurses should guide cancer patients to think about the meaning of life according to the factors affecting the sense of meaning of life.
