摘　　要：目的分析不同手术时机行股骨近端防旋髓内钉(PFNA)内固定对老年股骨粗隆间骨折的疗效、髋关节活动功能与生活质量的影响。方法随机选取灌云县人民医院2013年1月—2019年12月诊治的PFNA治疗的老年股骨粗隆间骨折患者120例,根据手术时机不同将其分成A组、B组。>3 d手术的患者共60例,将其纳入A组。≤3 d手术的患者共有60例,将其纳入B组。比较两组患者临床疗效、并发症、髋关节功能及生活质量。结果A、B组患者临床疗效差异比较有统计学意义(P<0.05)。A组患者并发症发生率高于B组(P<0.05)。术前A组、B组患者Harris评分差异比较无统计学意义,术后增加;其中A组患者Harris评分低于B组(P<0.05)。A组患者生理职能、躯体疼痛、情感职能、心理健康、社会职能、生理机能评分低于B组(P>0.05)。结论发病≤3 d给与老年股骨粗隆间骨折患者PFNA内固定术治疗能提搞临床疗效及髋关节功能,减少并发症。Objective To investigate internal proximal femoral nail antirotation(PFNA)fixation at different operative times on curative effect,hip joint motility and quality of life in elderly patients with intertrochanteric fracture.Methods 120 intertrochanteric fracture patients receiving PFNA in the hospital from January,2013 to December,2019 were randomly selected,and divided into two groups according to the operation time,with 60 cases in each group.Operation in group A was performed after injury of more than 3 days,while group B received operation within 3 days.The clinical efficacy,complication rate,hip function and quality of life were compared between groups.Results There was significant difference between groups in clinical effect(P<0.05),and the complication rate of group B was lower than that of group A(P<0.05).There was no difference in Harris scores between groups before operation(P>0.05),which were increased in both groups after operation,and were higher in group B than in group A(P<0.05).The scores of physiological function,bodily pain,emotional role,mental health,social function and physiological function of group A were lower than those of group B(P>0.05).Conclusion Treatment of PFNA internal fixation in elderly patients with intertrochanteric fractures within 3 days of injury can improve clinical efficacy and hip function,and reduce complications.
