摘 要:目的分析锁骨长度、骨性标记测量值以及导管理想长度的相关性,并建立经锁骨下静脉入路置入导管理想长度的多元方程。方法选取42例进行经锁骨下静脉输液港植入术的乳腺癌患者,分为左侧组与右侧组,分别为23与19例;探讨锁骨长度以及胸1椎下缘至气管隆突投影的距离与导管理想长度的相关性。结果左侧组患者植入式输液港理想置管长度(Y)与骨性标志测量值(X)之间以及锁骨长度之间无线性相关关系;右侧组植入式输液港理想置管长度(Y)与骨性标志测量值(X)以及锁骨长度之间存在线性相关关系,回归方程如为:Y=-1214+10X;Y=-93+3X。结论植入式输液港经右侧锁骨下静脉入路,借助于骨性标记测量值的回归方程对导管理想长度进行计算,有利于提高导管头端理想位置的准确率。Objective To analyze the correlation between the length of clavicle,the measured value of bony landmarks and the ideal length of catheter,and to establish a multivariate equation for the ideal length of the catheter inserted through subclavian vein approach.Methods 42 cases of breast cancer patients undergoing port implantation through subclavian vein were divided into the left group and the right group,including 23 and 19 cases respectively;the correlation between the length of the clavicle as well as the projection distance from the lower edge of thoracic vertebra to the tracheal carina and the ideal length of the catheter was discussed.Results There was no linear correlation between the ideal cathetering length of implantable venous access port(IVAP)of patients in the left group(Y)and the measured value of bony landmarks(X)as well as the length of clavicle,while there was a linear correlation between the ideal cathetering length of IVAP of patients in the right group(Y)and the measured value of bony landmarks(X)as well as the length of clavicle,with the regression equation as:Y=-1214+10 X;Y=-93+3 X.Conclusions The ideal length of the catheter was calculated using the regression equation of the measured value of bony landmarks based on the IVAP through right subclavian vein approach,which is helpful to improve the accuracy of the ideal position of the catheter tip.
