摘 要:目的探讨高频彩超联合超微血管成像(SMI)技术在不完全睾丸扭转中的诊断价值。方法回顾性分析42例经临床随访或手术证实为不完全睾丸扭转患者的临床资料,比较单纯使用高频彩超与高频彩超结合SMI技术诊断不完全睾丸扭转的准确性。结果42例患者中,高频彩超诊断为不完全睾丸扭转30例,准确率为71.4%;高频彩超联合SMI技术诊断为不完全睾丸扭转38例,准确率为90.5%;高频彩超联合SMI技术诊断不完全睾丸扭转的准确率高于高频彩超(P<0.05)。7例被高频彩超误诊为正常睾丸的患者中(健患两侧睾丸血流评级相同),4例由高频彩超联合SMI技术更正诊断为不完全睾丸扭转(患侧睾丸血管指数小于健侧);5例被高频彩超误诊为完全型睾丸扭转的患者中(患侧睾丸血流信号0级),4例由高频彩超联合SMI技术更正诊断为不完全睾丸扭转(患侧睾丸血管指数大于0,但小于健侧睾丸)。结论高频彩超结合SMI技术能为诊断不完全睾丸扭转提供更多重要的影像学信息,准确率高于高频彩超。Objective To explore the diagnostic value of high-frequency color Doppler ultrasound combined with superb microvascular imaging(SMI)technology for incomplete testicular torsion.Methods The clinical data of 42 patients with incomplete testicular torsion identified by clinical follow-up or surgery were retrospectively analyzed.High-frequency color Doppler ultrasound alone was compared with its combination with SMI technology in items of diagnostic accuracy rate for incomplete testicular torsion.Results Among the 42 patients,there were 30 cases of incomplete testicular torsion identified by high-frequency color Doppler ultrasound,with an accuracy rate of 71.4%;there were 38 cases of incomplete testicular torsion identified by high-frequency color Doppler ultrasound combined with SMI technology,with an accuracy rate of 90.5%;the accuracy rate of high-frequency color ultrasound combined with SMI technology was higher than that of high-frequency color Doppler ultrasound(P<0.05).Among seven patients misdiagnosed with normal testis identified by high-frequency color Doppler ultrasound(testes on healthy and affected sides presented same blood flow grading),four had a corrected diagnosis of incomplete testicular torsion revealed by high-frequency color Doppler ultrasound combined with SMI technology(testicular vascular index was smaller on affected side than on healthy side);among five patients misdiagnosed with complete testicular torsion identified by high-frequency color Doppler ultrasound(testis on affected side with blood flow signal of grade 0),four had a corrected diagnosis of incomplete testicular torsion revealed by high-frequency color Doppler ultrasound combined with SMI technology(testicular vascular index on affected side was greater than 0,but smaller than that on healthy side).Conclusion High-frequency color Doppler ultrasound combined with SMI technology can provide more important imaging information for the diagnosis of incomplete testicular torsion,exhibiting a higher accuracy rate than high-frequency col
