新型冠状病毒致病的中医认识和思考

摘　　要：通过分析新型冠状病毒感染者的临床表现,结合古籍和文献对于疫病的记录,对于新型冠状病毒的致病特点产生了几点中医认识和思考,本文主要从疫病概念及发病特点、病因病机的认识、舌诊的辨证分析、危重症的分析、治疗注意事项5个方面来阐述。从中医角度,新型肺炎属中医“疫病”范畴,其发病基本病机应是疫毒之邪外侵,肺脾受损,正气亏虚。多数伴有基础疾病的患者,可转为危重症,考虑与肺之化源绝和心神内闭、内闭外脱有关。另外,疫病诊治过程中应注重舌诊的辨证分析,其变化迅速,可提示疾病转归。治疗时应给邪气以出路,但需辨证应用下法,且谨慎应用补法。Based on analysis of clinical manifestations of corona virus disease 2019(COVID-19),statements from ancient books and published literature on pestilential diseases have been reviewed in this paper to sort out how traditional Chinese medicine interprets the pathogenic characteristics of COVID-19.This paper mainly expounds from five aspects:the concept and characteristics of pestilence,etiology and pathogenesis,pattern differentiation combined with tongue diagnosis,analysis of critical and severe cases,and considerations for treatment.From the perspective of traditional Chinese medicine,COVID-19 falls into the category of pestilential diseases.The basic pathogenesis of the disease is invasion of external pathogens leading to damage to the lung and spleen,and deficiency of the healthy qi.Most patients with underlying diseases tend to progress to critical and severe stages,which are related to the depletion of lung's source supply,the blockage of heart and spirit,ending up with internal blockage and external collapse.In addition,in the diagnosis and treatment of pestilence,tongue diagnosis should be highlighted as it can reflect the real-time progression of the disease.Pathogenic qi should be expelled in time,but purgation method should be applied based on pattern differentiation,and tonifying methods should be used with caution.
