摘　　要：目的探讨乳腺X线立体定位置入导丝引导术在乳腺微钙化中的临床应用价值。方法收集89例经钼靶发现的乳腺微钙化患者(BI-RADS4类及以上),临床需行手术切除或手术活检,患者均采用乳腺X线立体定位装置对乳腺微钙化灶进行导丝置入,引导快速切除病灶行病理检查,良性则单纯切除病灶,恶性则行保乳术或乳腺癌改良根治术。结果89例患者中87例均一次性导丝引导成功定位,病灶均完整切除,定位准确率达到97.8%。89例患者均进行手术活检和病理学诊断:浸润性癌2例(2.2%),原位癌5例(5.6%),乳头表皮腺癌1例(1%),余为良性病变。结论针对仅乳腺X线显示有微钙化灶的乳腺病变患者,乳腺X线立体定位导丝置入引导术操作便捷,并发症少,可成为临床手术中非常有效的定位方式,准确有效帮助乳腺病变的定性诊断,及早发现早期乳腺癌。Objective To explore the clinical application value of mammograms stereoscopic positioning guide wire guided surgery in breast microcalcification.Methods A total of 89 cases of breast microcalcification patients(BI-RADS 4 and above)who were found by mammography were collected.And surgical resection or biopsy was used to treat in clinical.All patients used mammograms stereoscopic positioning devices to insert guide wires for breast microcalcifications and to rapidly resect lesion for pathological examination.For benign,lesions were simply removed,while for malignant,breast-conserving surgery or modified radical mastectomy was performed.Results 87 of the 89 patients were successfully positioned with one-time guide wire guidance,the lesions were completely resected,and the positioning accuracy rate reached 97.8%.All 89 patients underwent surgical biopsy and pathological diagnosis:2 cases(2.2%)of invasive carcinoma,5 cases(5.6%)of carcinoma in situ,1 case(1%)of nipple epidermal adenocarcinoma,and the rest were benign lesions.Conclusion For patients with breast lesions that only mammograms show microcalcifications,mammograms stereoscopic positioning guide wire guided surgery is convenient with fewer complications.It may be an effective positioning method in clinical surgery that could accurately and effectively help to qualitatively diagnose breast lesions and early detect of early breast cancer.
