摘 要:[目的]应用有限元方法研究颈椎后路单开门不同微型钛板固定对颈椎稳定的影响。[方法]选择符合条件的颈椎后路单开门治疗的脊髓型颈椎病患者,利用Mimics、Geomagic Studio、Creo 4.0、Hypermesh构建颈椎后路单开门微型钛板固定的有限元模型,在Abaaus 6.1中设定材料力学参数、边界条件及载荷,测量颈椎在屈曲和后伸状态下的活动度和Von Mises应力。[结果]前屈、后伸状态下,连续固定模型与正常模型各节段的活动度均显著小于间断固定模型(P<0.05)。屈曲状态下,间断固定模型中固定椎板C3、C5、C7的Von Mises应力显著高于正常模型及连续固定模型(P<0.05),而非固定椎板C4、C6的Von Mises应力显著低于前两模型(P<0.05)。屈曲和后伸状态下,间断固定模型C3、C5、C7节段钛板的Von Mises应力均显著高于连续固定模型(P<0.05)。[结论]颈椎后路单开门连续微型椎板固定的稳定性优于间断固定。[Objective]To compare the stabilities of different mini lamina plate fixations in unilateral cervical open-door laminoplasty by a finite element analysis.[Methods]The finite element models of different mini lamina plate fixations in unilateral cervical open-door laminoplasty were created by using Mimics,Geomaeic studio,Creo 4.0 and HyperMesh software based on CT images obtained from patients who suffered from cervical spondylosis myelopathy.After the material parameters,boundary conditions and loads were set in Abaaus 6.1 software,the local range of motion(ROM),Von Mises stress on the laminae and plates were measured.[Results]In flexion and extension,the interrupted fixation model had significantly greater ROM than the normal control model and continuous fixation model(P<0.05).In flexion,the interrupted fixation model proved significantly greater Von Mises stresses on the laminae at C3,C5and C7that were fixed with the plates,whereas significantly less Von Mises stress on the C4and C6laminae that were not fixed with the plate than the normal control model and continuous fixation model(P<0.05).In addition,the interrupted fixation model proved significantly greater Von Mises stresses on the plates in both flexion and extension conditions than the continuous fixation model(P<0.05).[Conclusion]This finite element analysis demonstrated that the continuous mini lamina plate fixation is superior to in the interrupted fixation in biomechanical stability.
