男性SLE并发冠状动脉重度弥漫性病变1例报告并文献复习

摘　　要：目的探讨系统性红斑狼疮(SLE)并发严重冠状动脉病变病人的诊疗策略。方法回顾性分析1例青年男性SLE病人的临床资料,并重点复习近5年来国内外相关文献。结果结合病人临床表现、体格检查及辅助检查,诊断SLE并发冠状动脉重度弥漫性病变,考虑介入手术难度极大、风险极高,暂给予抗血小板、调脂、扩张血管等药物保守治疗,病人症状好转出院。结论SLE并发冠状动脉弥漫性病变病人血管脆性大,介入手术中冠状动脉破裂风险极高,故保守治疗可行。Objective To investigate the diagnosis and treatment strategy for patients with systemic lupus erythematosus(SLE)complicated by severe coronary artery disease.Methods A retrospective analysis was performed for the clinical data of a young male SLE patient,and a literature review was performed for related articles in China and foreign countries in the past 5 years.Results Based on the patient’s clinical manifestations,physical examination,and auxiliary examination,the patient was diagnosed with SLE complicated by severe diffuse coronary artery disease.Due to great difficulties and high risks of surgery,the patient was given conservative treatment including antiplatelet therapy,lipid regulation,and vasodilation,and the patient was discharged after the symptoms were improved.Conclusion Since patients with SLE complicated by diffuse coronary artery di-sease have highly fragile blood vessels,there may be a high risk of coronary artery rupture during interventional surgery.Conservative treatment is a feasible option.
