摘  要:目的探讨化瘀通阳方治疗溃疡性结肠炎(UC)的可能作用机制。方法24只Wistar大鼠随机分为空白组8只和模型复制组16只。模型复制组采用5%葡聚糖硫酸钠(DSS)溶液诱导UC大鼠模型。造模成功的10只大鼠随机分为化瘀通阳组和模型组,每组5只。化瘀通阳组予浓度为0.54g/ml化瘀通阳方2ml保留灌肠,空白组和模型组大鼠用等量的生理盐水灌肠,连续10天。干预结束后观察结肠组织病理,并采集大鼠粪便,进行肠道菌群宏基因测序分析。结果模型组大鼠结肠黏膜充血水肿,结构紊乱,黏膜下层可见炎性细胞浸润;化瘀通阳组大鼠结肠黏膜结构恢复正常,可见个别炎性细胞浸润,腺体增生活跃。与空白组比较,模型组大鼠结肠组织损伤指数(TDI)评分升高、属水平和种水平普雷沃氏菌属减少(P<0.01);与模型组比较,化瘀通阳组大鼠结肠TDI评分降低、属水平普雷沃氏菌属及种水平鼠乳杆菌增加、苯丙氨酸降解减少、赖氨酸生物合成增加(P<0.05或P<0.01)。Lefse分析模型组差异菌为梭菌属、Turicibacter、Faecalibaculum,化瘀通阳组差异菌群主要为乳杆菌属和Akkermansia。结论化瘀通阳方可以促进UC大鼠肠黏膜愈合,调节肠道菌群的丰度变化,其机制可能与乳杆菌属和Akkermansia的富集有关。Objective To explore the possible mechanism of Huayu Tongyang Formula(化瘀通阳方)in the treatment of ulcerative colitis(UC).Methods A total of 24 Wistar rats were randomly divided into a blank group of 8 rats and a model replication group of 16 rats.In the model replication group,5%dextran sodium sulfate(DSS)solution was used to induce UC rat model.The 10 successfully modeling rats were randomly divided into a Huayu Tongyang group and a model group,with 5 rats in each group.The Huayu Tongyang group was given a retention enema with concentration of 0.54 g/ml Huayu Tongyang Formula at a dose of 2 ml,and rats in the blank group and model group received the same amount of normal saline enema for 10 consecutive days.After the intervention,the colonic histopathology was observed,and rat feces were collected for metagenomic sequencing analysis of intestinal flora.Results The colonic mucosa congestion and endema,structural disorder,and inflammatory cell infiltration in the submucosa were found in the model group;the structure of the colonic mucosa of rats in the Huayu Tongyang group returned to normal,with individual inflammatory cell infiltration and active gland hyperplasia.Compared with the blank group,the colon tissue injury index(TDI)score of the model group increased,and the genus and species levels of Prevotella decreased(P<0.01).Compared with the model group,the rat colonic TDI score in the Huayu Tongyang group decreased,the genus level of Prevotella and the species level of Lactobacillus murine increased,the degradation of phenylalanine decreased,and the biosynthesis of lysine increased(P<0.05 or P<0.01).Lefse ana-lysis showed that the differential bacteria in the model group were Clostridium,Turicibacter,and Faecalibaculum,and the differential bacteria in the Huayu Tongyang group were mainly Lactobacillus and Akkermansia.Conclusion Huayu Tongyang Formula can promote the mucosa healing of UC and regulate the abundance change of intestinal flora,and its mechanism may be related to the enrichment of Lactobaci
