儿童重症腺病毒肺炎的预后特点及相关危险因素分析

摘　　要：目的探讨儿童重症腺病毒肺炎(SAP)的预后特点及其相关危险因素,为临床早期预防和治疗儿童SAP提供依据。方法回顾性分析156例SAP患儿资料,依据预后是否良好分为良好组(37例)和不良组(119例);分别采用单因素卡方检验和多因素Logistic回归法来筛选影响SAP预后的独立危险因素。结果 SAP患儿的预后不良发生率为23.7%。多因素分析结果显示,先天性心脏病(OR=5.387)、免疫功能低下(OR=2.732)、ARDS (OR=6.639)、肺外并发症(OR=2.333)、严重影像学改变(OR=2.012)等均为SAP患儿预后不良的危险因素。结论 SAP患儿具有较高的预后不良率,影响其预后的独立危险因素包括先天性心脏病等基础疾病、免疫功能低、ARDS、肺外较多并发症、肺部医学影像学改变等,应针对独立危险因素早期采取干预措施。Objective To explore the characteristics of prognosis and related risk factors in children with severe adenovirus pneumonia(SAP),provide a basis for clinical early prevention and treatment.Methods The data of 156 children with SAP were analyzed retrospectively,then the children were divided into good prognosis group(37 children)and bad prognosis group(119 children)according to prognosis;single factor Chi-square test and multivariate Logistic regression analysis were respectively used to screen the independent risk factors of prognosis of SAP children.Results The bad prognosis rate of SAP children was 23.7%.Multivariate Logistic regression analysis showed that congenital heart disease(OR=5.387),immunodeficiency(OR=2.732),ARDS(OR=6.639),extrapulmonary complications(OR=2.333),and severe imaging changes(OR=2.012)were the risk factors of bad prognosis in children with SAP.Conclusion The rate of bad prognosis is high in SAP children,the independent risk factors of prognosis of SAP children include congenital heart disease,immunodeficiency,ARDS,extrapulmonary complications,and severe imaging changes,intervention measures should be taken early according to independent risk factors.
