千金苇茎汤加减方辅助治疗支气管扩张症效果及对中医临床症状、肺功能和炎性因子改善情况

摘　　要：目的探讨千金苇茎汤加减辅助治疗支气管扩张症的临床效果。方法收集2018年3月—2019年9月诊治的80例支气管扩张症患者的临床资料,按治疗方法分为对照组和观察组,每组40例。对照组采用抗感染、排痰等常规治疗,观察组在对照组基础上采用千金苇茎汤加减方联合治疗。比较2组治疗前后中医证候评分、肺功能、血气分析、炎性因子水平变化情况,记录不良反应发生情况。结果观察组总有效率高于对照组(P<0.05);观察组中医证候评分优于对照组(P<0.05);观察组肺活量占预计值的百分比(FVC%pred)、第1秒用力呼气容积占预计值的百分比(FEVl%pred)、FEV1/FVC显著高于对照组(P<0.05);观察组动脉血氧分压和二氧化碳分压改善情况优于对照组,超敏C反应蛋白(hs-CRP)、白介素-4(IL-4)、IL-6水平低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论千金苇茎汤辅助治疗支气管扩张症具有良好疗效,能够明显改善患者症状,增强肺通气功能,降低气道炎症反应,且具有较高的用药安全性。Objective To explore clinical effect of modified Qianjin Weijing Decoction on bronchiectasis.Methods The clinical data of 80 patients with bronchiectasis diagnosed and treated from March 2018 to September 2019 were retrospectively analyzed.The patients were divided into control group and observation group according to different treatment methods,with 40 cases in each group.The control group was given conventional treatments including anti-infection,sputum excretion,while observation group was additionally treated with modified Qianjin Weijing Decoction on the basis of control group.The changes in TCM syndrome scores,lung function,blood gas analysis and inflammatory factors in both groups were compared,and the incidence of adverse reactions after treatment was recorded.Results The total response rate in observation group was significantly higher than that in control group(P<0.05),and TCM symptoms scores were higher than those in control group(P<0.05).The forced vital capacity as a percentage of predictive value(FVC%pred),forced expiratory volume in one second as a percentage of predictive value(FEVl%pred)and FEV1/FVC in observation group were significantly higher than those in control group(P<0.05).The partial arterial oxygen pressure(PO 2),arterial blood carbon dioxide partial pressure(PCO 2)in observation group were significantly better than those in control group,while high-sensitivity C-reactive protein(hs-CRP),interleukin-4(IL-4),and interleukin-6(IL-6)were significantly lower than those in control group(P<0.05).No significant differences were observed in the incidence rate of adverse reactions in both groups(P>0.05).Conclusion The modified Qianjin Weijing Decoction is effective in the treatment of bronchiectasis.It can significantly improve symptoms of patients,enhance pulmonary ventilation function,and reduce airway inflammation reactions with good medication safety.
