蝮蛇解毒汤联合乌司他丁对蝮蛇咬伤患者肝肾功能的保护作用

摘　　要：目的:观察蝮蛇解毒汤联合乌司他丁治疗对蝮蛇咬伤患者肝肾功能的保护作用。方法:将蝮蛇咬伤患者89例随机分为对照组45例与观察组44例。2组均给予乌司他丁注射治疗,观察组加用蝮蛇解毒汤治疗。观察比较2组治疗前后伤口肿胀、疼痛恢复情况,检测治疗前后炎症相关指标[白细胞计数(WBC)、C-反应蛋白(CRP)]、肝肾功能生化指标[谷草转氨酶(AST)、谷丙转氨酶(ALT)、尿素氮(BUN)、肌酐(Cr)]水平变化,并统计2组总体临床疗效。结果:总有效率观察组为86.4%,对照组为64.4%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患者伤口肿胀、疼痛程度均较治疗前明显改善(P<0.05),且观察组肿胀、疼痛程度改善较对照组更显著(P<0.05)。治疗后,2组患者WBC、CRP水平均较治疗前明显降低(P<0.05),且观察组WBC、CRP水平明显低于对照组(P<0.05)。治疗后,2组患者AST、ALT、BUN、Cr水平均较治疗前明显降低(P<0.05),且观察组上述各项指标明显低于对照组(P<0.05)。结论:蝮蛇解毒汤治疗蝮蛇咬伤可显著改善患者临床症状,减轻炎症反应,更好地保护肝肾功能,避免严重的脏器损伤。Objective:To observe the protective effect of Fushe Jiedu tang combined with ulinastatin on liver and kidney function in patients with agkistrodon bite. Methods: A total of 89 cases of patients with agkistrodon bite were randomly divided into the control group and the observation group, 45 and 44 cases in each group respectively. Both groups were treated with ulinastatin injection,and the observation group was additionally treated with Fushe Jiedu tang. Before and after treatment,the recovery of swelling and pain in the wound in the two groups was observed and compared;the changes in levels of inflammation related indexes including white blood cell count(WBC) and C-reactive protein(CRP) as well as biochemical indexes of liver and kidney function including aspartate aminotransferase(AST),alanine aminotransferase(ALT),blood urea nitrogen(BUN) and creatinine(Cr) were detected. The total clinical effect in the two groups was recorded. Results:The total effective rate was 86.4% in the observation group and 64.4% in the control group,the difference being significant(P<0.05). After treatment, the degrees of swelling and pain in the wound in the two groups were significantly improved when compared with those before treatment(P<0.05),and the improvement in the observation group was more significant than that in the control group(P<0.05). After treatment, the levels of WBC and CRP in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the levels in the observation group were significantly lower than those in the control group(P<0.05). After treatment,the levels of AST,ALT,BUN and Cr in the two groups were significantly decreased when compared with those before treatment(P<0.05), and the above indexes in the observation group were significantly lower than those in the control group(P<0.05).
