6例新型冠状病毒肺炎患者的流行病学与临床特征分析

摘　　要：目的:分析我院确诊的新型冠状病毒肺炎(COVID-19)患者的流行病学特点、临床特征和治疗结果,为疫情防控提供临床参考。方法:回顾性分析2020年1月20日至2020年4月1日,首都医科大学附属北京安贞医院确诊的6例COVID-19患者的流行病学特征、临床表现、辅助检查结果及治疗后转归。结果:观察期间共有6例疑似患者分别经朝阳区和北京市疾控中心病毒实验室核酸检测证实为COVID-19确诊病例,占同期疑似患者的15%(6/40)。6例患者(4例男性,2例女性)平均年龄54岁,4例有明确武汉旅游或居住史,1例有与COVID-19确诊患者密切接触史,1例无明确流行病学史。发病平均潜伏期2.3 d。6例患者均有发烧和肌肉酸痛、4例咳嗽、2例咽痛。血常规检查WBC总数均正常,1例淋巴细胞计数降低。所有患者流感筛查均阴性。4例患者血hs-CRP反应蛋白水平升高。6例患者均接受胸X线片检查,其中5例回报正常,1例胸X线片显示:双侧肺炎性表现,后经胸部CT扫描证实双肺渗出和磨玻璃影;1例胸X线片未回报异常但CT检查显示双肺磨玻璃表现。综合临床表现和影像学,4例患者诊断为轻型,2例为普通型。6例患者均转定点医院住院治疗并康复出院,平均住院时间为19.7 d。结论:早期COVID-19患者绝大多数有武汉旅游史或与确诊COVID-19患者密切接触;发热、咳嗽和肌肉酸痛为主要症状;本研究患者全部为轻型或普通型,经早发现、早诊断、早隔离和早治疗,预后良好。Objective:To delineate epidemiological and clinical characteristics of patients with coronavirus disease-19(COVID-19)associated pneumonia,which might shed initial lights on curtailing the pandemic.Methods:Retrospective analysis of epidemiological characteristics,clinical manifestation,laboratory examination of COVID-19 patients diagnosed in Beijing Anzhen Hospital,Capital Medical University,from January 20,2020-April 1,2020,and prognosis post comprehensive therapy in COVID-19-oriented hospital.Results:Six out of 40 suspected cases were confirmed to have COVID-19 infection based on real-time reverse transcriptase polymerase chain reaction testing from throat swab sample.Four male and two female patients had a mean age of 54.3 years.Four subjects reported recent traveling history to Wuhan either as local residence or tourists,while one reported close contact with confirmed cases prior to the diagnosis,despite one declined any obvious epidemical exposure.Average incubation duration was 2.3 days.Fever and muscular soreness were reported in all patients,and cough in 4,sore throat in 2.WBC were normal in all and one had reduced lymphocyte count.CRP was elevated in four patients,and no positive result for flu test.One case with normal X-ray imaging was verified to have typical manifestation of bilateral ground-glass opacity by CT scan,and one demonstrated infiltration and ground-glass opacity by both X-ray and CT scan,satisfying the criterion of common severity,while other patients had normal imaging(mild severity).All patients were transferred to state-nominated hospitals and treated comprehensively for an average duration of 19.7 days before discharge.Conclusions:The majority of early cases of COVID-19 pneumonia experienced Wuhan traveling and reported febrile,cough,muscle soreness.All patients surveyed only met the criterion of mild or common severity and achieved benign prognosis after early diagnosis,quarantine and treatment.
