自拟活血祛瘀方辨证加减对髋部骨折患者术后凝血功能的影响

摘　　要：目的:探讨自拟活血祛瘀方辨证加减对髋部骨折患者术后凝血功能的影响。方法:选取2018年11月-2019年11月间我院收治的髋部骨折患者75例,随机将其分为研究组(38例)和对照组(37例)。对照组给予常规药物治疗,研究组在此基础上使用自拟活血祛瘀方辨证加减治疗。比较两组患者临床疗效、炎症反应、凝血功能、下肢深静脉血栓(Deep Vein Thrombosis,DVT)及肺栓塞发生情况。结果:干预后研究组有效率显著高于对照组,差异具有统计学意义(P<0.05)。干预前两组肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α),白细胞介素(Interleukin,IL)-6、IL-10、C反应蛋白(C-reactive Protein,CRP)比较,差异无统计学意义;干预后研究组TNF-α、IL-6、CRP显著低于对照组(P<0.05),同时研究组IL-10显著高于对照组(P<0.05)。两组患者干预前后凝血功能比较,干预前两组患者血浆纤维蛋白原(Fibrinogen,FIB)、凝血酶原时间(Prothrombin Time,PT)、活化部分凝血活酶时间(Activated Partial Thromboplastin Time,APTT)、D-二聚体(D-Dimer)比较,差异无统计学意义(P>0.05);干预后研究组PT、APTT显著高于对照组,同时研究组FIB、D-Dimer显著低于对照组(P<0.05)。干预后研究组DVT、肺塞栓显著低于对照组(P<0.05)。结论:通过自拟活血祛瘀方辨证加减对髋部骨折患者术后治疗,能有效降低炎症反应,缓解血液高凝状态,减少血栓发生,安全可靠,且价格便宜,值得临床推广。Objective:To explore the effects of the Huoxue Quyu prescription(活血祛瘀方)on coagulation function in patients with hip fracture after operation.Methods:75 cases were divided into the study group(38 cases)and control group(37 cases).The control group was given conventional treatment,and the study group was treated with the Huoxue Quyu prescription more.Results:The effective rate in the study group was significantly higher than the control group(P<0.05).TNF-α,IL-6 and CRP in the study group were significantly lower(P<0.05),and IL-10 was significantly higher than the control group(P<0.05).PT and APTT in the study group were significantly higher,and FIB and D-Dimer in the study group were significantly lower than the control group(P<0.05).DVT and pulmonary embolism in the study group were significantly lower than the control group(P<0.05).Conclusion:The Huoxue Quyu prescription can effectively reduce inflammatory reaction,relieve blood hypercoagulability,reduce thrombosis,is safe,reliable and cheap,and is worthy of clinical promotion.
