振幅整合脑电图在儿童先天性脑积水中的应用研究

摘　　要：目的研究振幅整合脑电图(aEEG)在先天性脑积水患儿中的应用价值。方法选取2015年1月至2018年6月该院收治的50例先天性脑积水患儿及20例健康儿童,检测其aEEG异常率,对其结果进行分析,并对脑积水患儿在术后3、6个月分别对其智能发育指数(MDI)、运动发育指数(PDI)及发育商(DQ)进行评分,进一步分析aEEG与先天性脑积水预后之间的关系,同时观察患儿的临床疗效及预后。结果 20例健康儿童aEEG总异常率为5.0%,50例先天性脑积水患儿的aEEG总异常率为92.0%,差异有统计学意义(χ2=49.013,P<0.05)。轻度、中度、重度脑积水的异常率分别为88.9%、92.9%、100.0%,脑积水程度越高,aEEG异常率越高,差异有统计学意义(P<0.05)。术后3个月,aEEG异常程度越重,其术后MDI、PDI、DQ评分越低(P<0.05)。术后6个月,不同aEEG结果患儿MDI、PDI评分比较,差异均无统计学意义(P>0.05);而aEEG异常程度越重,其术后DQ评分越低(P<0.05)。结论 aEEG对反映先天性脑积水患儿脑功能状态有较强的敏感性,并对其预后判断有较高的参考价值。Objective To investigate the value of amplitude integrated eletroencephalograpy(aEEG)in the children with congential hydrocephalus.Methods A total of 50 cases of children with congenital hydrocephalus and 20 cases normal children admitted to Leshan People′s Hospital from January 2015 to June 2018 were selected to detect the abnormal rate of aEEG,and analyze the results.The mental development index(MDI),psychomotor development index(PDI)and developmental quotient(DQ)were scored 3,6 months after surgery in children with congenital hydrocephalus.The relationship between aEEG and prognosis of congenital hydrocephalus was further analyzed;at the same time,the clinical efficacy and prognosis of the children were observed.Results The total abnormal rate of aEEG in 20 normal children was 5.0%,and in 50 children with congenital hydrocephalus was 92.0%,the difference between the two groups was statistically significant(χ2=49.013,P<0.05).The abnormal rates of mild,moderate and severe hydrocephalus were 88.9%,92.9%and 100.0%,respectively.The degree of hydrocephalus was the higher,the aEEG abnormality rate was the higher,the difference was statistically significant(P<0.05).After 3 months of follow-up,the abnormality was the more severe,the postoperative MDI,PDI,DQ scores were the lower(P<0.05).After 6 months of follow-up,comparison of MDI,PDI scores among children with different aEEG results showed no statistically significant differences(P>0.05),but the abnormality was the more severe,the postoperative DQ score was the lower(P<0.05).Conclusion aEEG has a strong sensitivity to reflect the cerebral functional status of children with congenital hydrocephalus,and it has high reference value for its prognosis.
