摘　　要：目的为相似地区同级医院药事管理提供参考。方法采用失效模式和效应分析(FMEA)法分析西藏自治区尼玛县人民医院药品管理全环节中的潜在风险,并制订改进措施。结果医院在药品采购、储存、使用和管理等环节存在21项潜在失效模式,其中高风险失效模式13项。实施改进措施后,医院药事相关风险总体降低81.37%,20项失效模拟计算风险顺序数(RPN)显著减小,各项差异均有统计学意义(P<0.05)。结论通过FMEA法,可前瞻性地发现管理漏洞和隐患,不断完善制度和研究改进措施,切实提高藏西北县级医院药事管理水平,实现全方位药品质量管理。Objective To provide reference for the pharmaceutical management of hospitals at the same level in similar areas.Methods The failure mode and effect analysis(FMEA)method was used to analyze the potential risks in the whole process of drug management in Nima County People’s Hospital,and to develop improvement measures.Results The results of the study showed a total of 21 failure modes in links of drug procurement,storage,use and management,including 13 high risk failure modes.After drawing up the improvement measures of management,risk reduced by 81.37%,20 items of risk priority number(RPN)declined significantly,and the differences were statistically significant(P<0.05).Conclusion Through the FMEA method,it is possible to prospectively discover management loopholes and hidden dangers,continuously improve the system and research improvement measures,effectively improve the pharmaceutical management level of county-level hospitals in northwestern Tibet,and realize comprehensive drug quality management.
