内膜下血管成形术联合西洛他唑治疗动脉粥样硬化闭塞症临床研究

摘　　要：目的探讨内膜下血管成形术(SIA)联合西洛他唑治疗动脉粥样硬化闭塞症(ASO)的临床疗效及对患者总抗氧化能力(T-AOC)和谷胱甘肽过氧化物酶(GSH-Px)水平的影响。方法选取医院2016年2月至2019年4月收治的下肢ASO患者120例,按住院编号随机分为观察组和对照组,各60例。两组患者均予SIA手术,观察组患者加服西洛他唑片,治疗4周。结果观察组总有效率为83.33%,显著高于对照组的51.67%(P<0.05);与对照组比较,观察组患者治疗后的全血高切还原黏度(HRV)、血浆黏度(PV)及肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)、白细胞介素8(IL-8)水平均显著降低,全血低切还原黏度(LRV)水平显著升高,T-AOC和GSH-Px活性均显著增强(P<0.05);两组均未见明显不良反应。结论SIA联合西洛他唑治疗ASO,可增强GSH-Px活性及T-AOC,改善炎性因子及血液流变学指标。Objective To observe the clinical efficacy of subintimal angioplasty(SIA)combined with cilostazol in the treatment of atherosclerosis obliterans(ASO)and its effect on the levels of total antioxidant capacity(T-AOC)and glutathione peroxidase(GSH-Px).Methods A total of 120 patients with lower extremity ASO who were treated in our hospital from February 2016 to April 2019 were selected and randomly divided into the observation group and control group according to their hospital number,60 cases in each group.Both groups were treated with SIA,and the observation group was treated with cilostazol on the basis of SIA.Both groups were treated for 4 weeks.Results The total effective rate in the observation group was 83.33%,which was significantly higher than51.67%in the control group(P<0.05).Compared with the control group,the observation group had high shear reduction viscosity(HRV),plasma viscosity(PV)and tumor necrosis factor-α(TNF-α),interleukin 6(IL-6),and interleukin 8(IL-8)levels were significantly reduced,and whole blood low-cut reduced viscosity(LRV)levels significantly increased,T-AOC and GSH-Px activities were significantly enhanced(P<0.05);no obvious adverse reactions were seen in both groups.Conclusion SIA combined with cilostazol in the treatment of ASO can significantly enhance the activity of GSH-Px and T-AOC,and significantly improve inflammatory factors and hemorheological indicators.
