中西医抗结核药联合经支气管镜冷冻治疗气管支气管结核临床观察

摘　　要：目的研究中西医抗结核药联合经支气管镜冷冻治疗气管支气管结核(EBTB)的临床疗效。方法选取医院2017年8月至2019年6月收治的EBTB患者92例,随机分为观察组和对照组,各46例。两组患者均予规范化全身抗结核治疗,并行经支气管镜冷冻治疗;观察组患者加用自拟肺痨汤;均持续治疗3个月。结果观察组3个月痰菌转阴率、胸部CT总有效率、支气管镜评定总有效率分别为90.24%,86.96%,95.65%,显著高于对照组的71.79%,63.04%,78.26%(P<0.05);观察组6个月痰菌转阴率为95.12%,与对照组的84.62%相当(P>0.05)。结论中西医抗结核药联合经支气管镜冷冻治疗溃疡坏死型和肉芽增殖型EBTB,可使痰菌早期转阴,促进肺部病灶吸收,并可改善气道狭窄状态。Objective To observe the efficacy of transcatheter interventional cryotherapy combined with Chinese herbs in the treatment of endobronchial tuberculosis(EBTB).Methods A total of 92 patients with EBTB admitted to the hospital from August 2017 to June2019 were selected and randomly divided into the observation group and control group,46 cases in each group.Both groups received standard systemic chemotherapy and transcatheter interventional cryotherapy.On this basis,the observation received Chinese herbs therapy of self-designed Feilao Decoction.Both groups were continuously treated for 3 months.Results The sputum negative conversion rate after 3 months,imaging efficiency and microscopic efficiency of the observation group were 90.24%,86.96%and 95.65%,respectively,which were significantly higher than 71.79%,63.04%,and 78.26%in the control group(P<0.05).The sputum negative conversion rate in the observation group was 95.12%after 6 months,which was equivalent with 84.62%in the control group(P>0.05).Conclusion Combination of transcatheter interventional cryotherapy and Chinese herbs in treating EBTB can make the sputum negative transformed early,improve lesions absorption and improve tratheobronchial stenosis.
