摘　　要：目的探讨两指法转胎头对持续性枕后位产妇生命体征及新生儿结局的影响。方法选取2018年2月至2019年11月于本院就诊的持续性枕后位产妇148例,随机分为对照组和研究组各74例。对照组采用传统转胎头法治疗,研究组采用二指法转胎头治疗。观察两组产妇分娩结局、产妇生命体征、新生儿结局。结果研究组转位成功率、胎方位纠正率、自然分娩率高于对照组,剖宫产率低于对照组(P<0.05)。研究组血压(BP)、血氧饱和度(SpO 2)、心率(HR)均高于对照组(P<0.05)。研究组新生儿双顶径、1min新生儿Apgar评分、胎心率、胎心基线变异减少率均高于对照组,新生儿窒息率低于对照组(P<0.05)。结论两指法转胎头能够稳定持续性枕后位产妇的生命体征,改善产妇分娩结局及新生儿结局。Objective To study the effects of two-finger-rotation on maternal vital signs and neonatal outcomes in persistent occipital posterior position.Methods A total of 148 women with persistent occipital posterior position admitted to our hospital from February 2018 to November 2019 were selected and randomly divided into control group and study group,with 74 cases in each group.The control group was treated with traditional fetal head method,and the study group was treated with two-finger-rotation.The outcomes of delivery,maternal vital signs and newborn were observed.Results The success rate of translocation,the correction rate of fetal position and the rate of natural delivery in the study group were higher than those in the control group,and the cesarean section rate was lower than that in the control group(P<0.05).BP,SpO 2 and HR were higher in the study group than those in the control group(P<0.05).The double-peak diameter,Apgar score,fetal heart rate and baseline variance reduction rate in the study group were higher than those in the control group,and the neonatal asphyxia rate was lower than that in the control group(P<0.05).Conclusion Two-finger-rotation can stabilize the vital signs of postoccipital women and improve the outcome of delivery and newborn.
