摘　　要：目的探讨术前口咽部不同预处理方案对全麻气管插管术后患者口腔定居菌群及呼吸道感染的影响。方法选择本院2018年3月至2019年12月收治的80例全麻气管插管患者,随机分为对照组和观察组各40例。对照组术前给予生理盐水漱口,观察组术前给予活性银离子抗菌液漱口。观察两组患者细菌培养半定量结果、定植菌群菌量及拔管后呼吸道感染情况。结果观察组细菌半定量、革兰氏阳性球菌和革兰氏阴性杆菌量、呼吸道感染发生率均低于对照组(P<0.05)。结论全麻气管插管术前口咽部给予活性银离子抗菌液漱口,可降低术后口腔菌群分布,减少革兰氏阳性球菌和革兰氏阴性杆菌分泌,降低口咽定居菌群向肺部蔓延感染的发生率。Objective To explore the effects of different pretreatment methods of oropharynx before surgery on oral colonization flora and respiratory tract infection in patients with general anesthesia.Methods Eighty patients undergoing general anesthesia and tracheal intubation who were treated in our hospital from March 2018 to December 2019 were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with normal saline gargle before surgery and the observation group was treated with mouthwash with active silver ion antibacterial solution before surgery.The semi-quantitative results of bacterial culture,colony flora and respiratory infection after extubation were observed in two groups.Results The semi-quantitative bacteria,the amount of Gram-positive cocci and Gram-negative bacteria,incidence of respiratory tract infection in the observation group were lower than those in the control group(P<0.05).Conclusion Gavage of active silver ion antibacterial solution in the oropharynx before general anesthesia and tracheal intubation can reduce the oral flora distribution,reduce the secretion of gram-positive cocci and gram-negative bacteria,and reduce the oropharynx Incidence of colonized flora spreading to the lungs.
