摘　　要：目的探讨阴道用乳酸菌对高度宫颈上皮内瘤变(HSIL)患者术后HPV转归的影响。方法HSIL患者190例术后均分为试验组和对照组。试验组患者非经期使用阴道用乳酸菌,每晚1粒,连续治疗3个月;对照组不使用药物干预。检测并比较两组治疗前后阴道微生态和术后6、12个月HPV转阴率。结果治疗后试验组阴道炎症、pH值异常、菌群失调、清洁度异常、菌群密集异常及菌群多样性异常比例均低于对照组(P<0.05),且均低于治疗前(P<0.05)。试验组术后6、12个月HPV转阴率分别为62.11%、82.11%,高于对照组的42.11%、60.00%(P<0.05)。结论阴道用乳酸菌通过调整阴道微生态,提高HSIL患者术后HPV转阴率,从而降低宫颈病变复发。Objective To investigate the effect of vaginal lactobacillus on postoperative HPV outcome in the patients with high-grade cervical intraepithelial lesion(HSIL).Methods A total of 190 patients with HSIL after operation was randomly divided into two groups with 95 cases each.Group A during non-menstrual period was treated with vaginal lactobacillus,one tablet per night for 3 months.Group B was not given vaginal lactobacillus as the control.Vaginal microecology was detected and compared between the two groups before and after treatment.HPV negative conversion rate was compared between the two groups in the 6 th and 12 th month after treatment.Results After treatment,the percentages of the patients with vaginal inflammation,abnormal pH value,bacterial imbalance,cleanliness,bacterial density and bacterial diversity in group A were lower than those before treatment and those in group B(P<0.05).The HPV negative conversion rates in the 6 th and 12th month after treatment were higher in group A than those in group B(62.11%vs.42.11%and 82.11%vs.60.00%)(P<0.05).Conclusion By adjusting the vaginal microecology,vaginal lactobacillus can improve the negative conversion rate of HPV and reduce the recurrence of cervical lesions in the patients with HSIL.
