摘　　要：目的研究基于失效模式及效应分析(healthcare failure mode effect analysis,HFMEA)的环节管理模式对新生儿卡介苗接种安全的影响。方法2017年1-12月,便利抽样法选择在某院出生的1231名新生儿为对照组。2018年1-12月,同法选择在该院出生的1287名新生儿为观察组。对照组遵循以往新生儿接种管理方法,实施常规接种护理。观察组采用基于HFMEA的环节管理模式进行接种安全管理。比较两组新生儿风险优先指数(risk priority number,RPN)、接种不良反应发生情况以及家属对卡介苗安全性感知情况。结果观察组新生儿卡介苗接种的RPN值均低于对照组。观察组患者不良反应发生率为0.3%(4例)明显低于与对照组的1.4%(17例),差异有统计学意义(χ^2=7.467,P=0.006)。与对照组比较,观察组新生儿家属对卡介苗安全性感知情况明显提升,差异有统计学意义(P<0.05)。结论基于HFMEA的环节管理模式可以降低新生儿卡介苗接种环节风险性、降低不良反应发生率,提高家属对卡介苗的安全性感知,值得推广。Objective To study the effects of the link management model based on healthcare failure mode and effect analysis(HFMEA)on the safety of BCG vaccination in neonates.Methods A total of 1287 newborns in our hospital from January to December 2018 were selected as the observation group and 1231 newborns in our hospital from January to December 2017 were selected as the control group by convenience sampling method.The observation group adopted the link management model based on HFMEA scheme for safe inoculation management,while the control group still followed the previous neonatal inoculation management methods and carried out routine inoculation nursing.The RPN values,the incidence of adverse reactions and the safety perception of BCG vaccine were compared between the two groups.Results After intervention,the RPN value of observation group was lower than that of control group.The incidence of adverse patients of observation group(0.3%,4)was less than that of control group(1.4%,12),and the difference was statistically signficant(P<0.05),the safety perception of BCG vaccination was improved in the observation group and the control group,the differences were statistically significant(P<0.05).Conclusions The link management model based on HFMEA can reduce the risk of BCG vaccination,reduce the incidence of adverse reactions,and improve the safety perception of BCG vaccine.
