从量效关系探讨《伤寒杂病论》半夏用药规律

摘　　要：目的分析《伤寒杂病论》中半夏的量效关系及配伍用药规律,为临床应用提供依据。方法依据《伤寒论》《金匮要略》原文建立数据库,采用SPSS23.0统计软件对半夏用量与相关因素之间的关系进行分析。结果纳入含半夏方剂36首。半夏每服量与药味数有相关性(P<0.001,r=-0.700),与处方加水量、煎煮后水量、每服水量、服用次数、每服整方量无明显相关性(P>0.05)。半夏作为君药时,其每服量大(P=0.019)、药味数少(P<0.001)、煎煮后水量小(P=0.012)。半夏配伍大枣、黄芩、甘草时,配伍药物间用量存在相关关系(P<0.001,P<0.001,P=0.029),而与生姜、人参、炙甘草配伍时未发现相关关系(P>0.05),半夏配伍黄芩时其每服量小(P=0.011)。结论半夏在《伤寒杂病论》中的应用具有鲜明的量效关系,其用量规律可为临床应用及量效关系研究提供依据。Objective To analyze the dosage-effect relationship and compatibility medication rules of Pinelliae Rhizoma in Shang Han Za Bing Lun;To provide references for clinical medication.Methods Database according to original texts of Shang Han Lun and Jin Gui Yao Lue was established.The dosage-effect relationship of Pinelliae Rhizoma and the related factors were analyzed by SPSS 23.0.Results This study included 36 prescriptions containing Pinelliae Rhizoma.A single dosage of Pinelliae Rhizoma related to the number of medicine(P<0.001,r=-0.700),and the prescription of water,water after decoction,water of each prescription and the frequency had no significant correlation with a single dosage(P>0.05).While Pinelliae Rhizoma as a monarch medicine,it had a large a single dosage(P=0.019),less number of medicine(P<0.001),less water after the decoction(P=0.012).When Pinelliae Rhizoma was mixed with Jujubae Fructus,Scutellariae Radix and Glycyrrhizae Radix et Rhizoma,there was a correlation with the dosage of the two compatible medicines(P<0.001,P<0.001,P=0.029),but no correlation was found when mixed with Zingiberis Rhizoma Recens,Ginseng Radix et Rhizoma and Glycyrrhizae Radix et Rhizoma Praeparata cum Melle(P>0.05),and when mixed with Scutellariae Radix,each dosage was small(P=0.011).Conclusion Pinelliae Rhizoma has a distinct dosage-effect relationship in its application in Shang Han Za Bing Lun.Its use and compatibility are strict and flexible,and its dosage law can provide basis for clinical application and dosage-effect relationship research.
