全椎板切除脊柱内固定治疗老年退行性腰椎管狭窄症患者的疗效观察

摘　　要：目的:观察全椎板切除脊柱内固定治疗老年退行性腰椎管狭窄症患者的临床效果。方法:选取2017年12月—2020年1月我院老年退行性腰椎管狭窄症患者160例,所有患者均接受全椎板切除脊柱内固定治疗,以12个月为术后随访时长,探讨实际治疗效果。结果:相较于术前,术后3个月、12个月VAS评分更低,JOA评分更高,椎管直径更大,差异有统计学意义(P<0.05);相较于术后3个月,术后12个月V A S评分更低,JO A评分更高差异有统计学意义(P<0.05)。治疗优良率为91.25%;术后并发症率为2.5%。结论:在老年退行性腰椎管狭窄症治疗时,更推荐应用全椎板切除脊柱内固定方法,可减轻下腰痛程度,使椎管直径增大,手术安全系数较高,整体治疗效果明确。Objective To study the clinical effect of total laminectomy and spinal internal fixation in the treatment of senile degenerative lumbar spinal stenosis.Methods 160 elderly patients with degenerative lumbar spinal stenosis in our hospital were collected from December 2017 to January 2020.All patients received total laminectomy and spinal internal fixation.The follow-up period was 12 months to explore the actual therapeutic effect.Results Compared with before operation,the Vas score was lower at 3 months and 12 months after operation,the Joa score was higher,the diameter of vertebral canal was larger,the difference was significant(P<0.05).Compared with 3 months after operation,VAS score is lower and JOA score is higher at 12 months after operation,with significant difference(P<0.05).The excellent and good rate of treatment was 91.25%.The postoperative complication rate was 2.5%.Conclusion In the treatment of senile degenerative lumbar spinal stenosis,total laminectomy and spinal internal fixation are more recommended,which can reduce the degree of low back pain,increase the diameter of spinal canal,have higher safety factor of operation,and have definite overall treatment effect.[Key Words]Total laminectomy and spinal internal fixation;Old age;Degeneration;Lumbar spinal stenosis.
